针灸联合补阳还五汤加减治疗急性脑梗死后偏瘫患者的效果

Effects of acupuncture combined with Buyang Huanwu decoction in treatment of hemiplegia patients after acute cerebral infarction

目的:观察针灸联合补阳还五汤加减治疗急性脑梗死后偏瘫患者的效果。方法:选取2019年6月至2020年6月该院收治的72例急性脑梗死后偏瘫患者进行前瞻性研究,按照随机数字表法将其分为观察组和对照组各36例。对照组采用补阳还五汤加减治疗,观察组在对照组基础上联合针灸治疗,比较两组治疗效果、治疗前后下肢功能[Fugl-Meyel运动功能量表(FMA)]评分、日常活动能力[Barthel指数]评分、中医证候积分和不良反应发生率。结果:观察组治疗总有效率为97.22%(35/36),高于对照组的77.78%(28/36),差异有统计学意义(P<0.05);治疗后,两组FMA下肢功能评分和Barthal评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);治疗后,两组中医证候积分均低于治疗前,观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:针灸联合补阳还五汤治疗急性脑梗死后偏瘫患者可提高治疗总有效率、下肢运动功能评分和日常活动能力评分,降低中医证候积分,其效果优于单纯补阳还五汤加减治疗。

Objective:To observe effects of acupuncture combined with Buyang Huanwu decoction on hemiplegia patients after acute cerebral infarction.Methods:A prospective study was conducted on 72 patients with hemiplegia after acute cerebral infarction admitted to the hospital from June 2019 to June 2020.According to the random number table method,they were divided into observation group and control group,36 cases in each group.The control group was treated with Buyang Huanwu decoction,while the observation group was treated with acupuncture on the basis of that of the control group.The therapeutic effect,the lower limb function[Fugl-Meyel assessment(FMA)]score,the activities of daily living(Barthel index)score,the TCM syndrome score,and the incidence of adverse reactions were compared between the two groups.Results:The effective rate of treatment in the observation group was 97.22%(35/36),which was higher than 77.78%(28/36)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the FMA lower limb function scores and the Barthal scores of the two groups were higher than those before the treatment,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the TCM syndrome scores of the two groups were lower than those before the treatment,that in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Acupuncture combined with Buyang Huanwu decoction can improve the effective rate of treatment,the lower limb motor function score and the activities of daily living scores,and reduce the TCM syndrome scores in the patients with hemiplegia after acute cerebral infarction.Moreover,it is superior to single Buyang Huanwu decoction treatment.
