PDCA循环护理在血液透析患者中的应用效果

Application effects of PDCA cycle nursing in hemodialysis patients

目的:观察计划-执行-检查-纠正(PDCA)循环护理在血液透析患者中的应用效果。方法:选取2019年6月至2019年12月该院收治的59例血液透析患者作为对照组,采用常规护理;选取2020年1月至2020年7月该院收治的55例血液透析患者作为观察组,采用PDCA循环护理。比较两组护理前后动静脉内瘘静脉端血流动力学指标(头静脉内径、头静脉血流量)水平、内瘘功能优良率和并发症发生率。结果:护理4、12周后,两组头静脉内径和头静脉血流量均低于护理前,但观察组高于对照组,差异有统计学意义(P<0.05);观察组内瘘功能优良率为78.18%,高于对照组的61.02%,差异有统计学意义(P<0.05);观察组并发症发生率为9.09%,低于对照组的23.73%,差异有统计学意义(P<0.05)。结论:PDCA循环护理应用于血液透析患者可提高动静脉内瘘静脉端血流动力学指标水平和内瘘功能优良率,降低并发症发生率,优于常规护理效果。

Objective:To observe application effects of Plan-Do-Check-Act(PDCA)cycle nursing in hemodialysis patients.Methods:A total of 59 patients who underwent hemodialysis in the nephrology department of the hospital from June 2019 to December 2019 were selected as the control group and received routine nursing.A total of 55 patients undergoing hemodialysis from January 2020 to July 2020 were selected as the observation group,and PDCA cycle nursing was used.The hemodynamic index levels(cephalic vein diameter,cephalic vein blood flow)of the venous end of the internal arteriovenous fistulas,the excellent and good rate of internal fistula function,and the incidence of complications were compared between the two groups before and after the nursing.Results:4 and 12 weeks after the nursing,the cephalic vein diameter and cephalic vein blood flow in the two groups were lower than those before the treatment,but those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The excellent and good rate of internal fistula function in the observation group was 78.18%,which was higher than 61.02%in the control group,and the difference was statistically significant(P<0.05).Further,the total incidence of complications in the observation group was 9.09%,which was lower than 23.73%in the control group,and the difference was statistically significant(P<0.05).Conclusions:The PDCA circulation nursing for the hemodialysis patients can improve the hemodynamic index levels of the venous end of the arteriovenous fistula and the excellent and good rate of internal fistula function,and reduce the incidence of complications.Moreover,it is superior to the routine nursing.
