基于决策树模型产后恶露不绝药物治疗方案评价

摘要 目的 分析五加生化胶囊、坤宁口服液和养血化瘀合剂治疗产后恶露不绝的有效性和经济性,为临床选择最佳药物治疗方案提供依据。方法 采用回顾分析法,收集2019年6月—2020年6月南京医科大学附属妇产医院/南京市妇幼保健院收治的产后恶露不绝患者278例,根据治疗方法不同分为A组(五加生化胶囊组)114例、B组(坤宁口服液组)86例、C组(养血化瘀合剂组)78例。采用决策树模型对上述3组治疗方案进行成本效果比和增量成本效果比分析。结果 3组患者治疗方案总有效率分别为92.98%、84.88%、94.87%,差异有统计学意义(P<0.05)。并且在治疗过程中3组方案均无严重不良反应发生。C组养血化瘀合剂组成本—效果比(CER)相比其他2组较低。结论 养血化瘀合剂治疗产后恶露不绝的效果优于五加生化胶囊和坤宁口服液,且更具有经济性。 Objective Using the analysis method of pharmacoeconomics,to discusses the best drug treatment plan of three kinds of postpartum lochiorrhea:Wujia Shenghua capsule,Kunning oral liquid and Yangxue Huayu mixture.Methods A retrospective analysis was conducted on 278 cases of postpartum lochiorrhea patients in the Women′s Hospital of Nanjing Medical University/Nanjing Maternity and Child Health Care Hospital from June 2019 to June 2020.According to different treatment methods,they were divided into group A(Wujia Shenghua capsule group)114 cases,group B(Kunning oral liquid group)86 cases,and group C(Yangxue Huayu mixture group)78 cases.The cost-effectiveness ratio and incremental cost-effectiveness ratio of the three schemes were analyzed by decision tree method.Results The total effective rates of the three groups were 92.98%,84.88%and 94.87%respectively,with statistically significant difference(P<0.05).No serious adverse reactions occurred in the three groups during treatment.The cost-effectiveness ratio(CER)of Yangxue Huayu mixture group(group C)was lower than the other two groups.Conclusion The effect of Yangxue Huayu mixture is better than Wujia Shenghua capsule and Kunning oral liquid in treating postpartum lochiorrhea,and it is more economical.
