守中定眩汤加减方治疗后循环缺血性眩晕的临床效果

Clinical effect of modified Shouzhong Dingxuan decoction in the treatment of posterior circulation ischemic vertigo

目的观察守中定眩汤加减方治疗后循环缺血性眩晕(PCIV)的临床效果。方法选取2020年1月—2022年2月广州中医药大学第一附属医院白云医院收治的PCIV患者120例,依据随机数字表法分为定眩组(n=60)和三联组(n=60)。三联组患者给予阿司匹林、阿托伐他汀、倍他司汀三联治疗,定眩组在三联组基础上给予守中定眩汤加减方治疗,2组均治疗14 d。比较2组患者治疗效果,治疗前后脑血流动力学指标[椎基底动脉血流速度]及不良反应。结果定眩组患者治疗总有效率为96.67%,高于三联组的83.33%(χ^(2)=5.926,P=0.015);治疗14 d后,2组患者左椎、右椎、基底动脉血流速度均高于治疗前,且定眩组高于三联组(P均<0.01);三联组与定眩组不良反应总发生率比较差异无统计学意义(6.67%vs.10.00%,χ^(2)=0.436,P=0.509)。结论守中定眩汤加减方治疗PCIV可提高临床疗效,改善患者脑血流动力学指标,且安全性高,值得推广与应用。

Objective To observe the clinical effect of modified Shouzhong Dingxuan decoction in the treatment of posterior circulation ischemic vertigo(PCIV).Methods One hundred and twenty patients with PCIV in Baiyun Hospital of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from January 2020 to February 2022 were selected,and they were divided into fixed dazzle group(n=60)and triple combination group(n=60)according to the random table.The patients in triple combination group were given aspirin,atorvastatin and betastine combined treatment,the patients in fixed dazzle group were given Shouzhong Dingxuan decoction plus or minus treatment on this basis,both groups were treated for 14 days.The therapeutic efficacy,cerebral hemodynamics[vertebrobasilar artery flow velocity]and adverse reactions between the two groups were compared.Results The total effective rate of patients in the fixed dazzle group was 96.67%,higher than 83.33%in the triple combination group(χ^(2)=5.926,P=0.015).After 14 days of treatment,the blood velocity of left vertebra,right vertebra and basilar artery of two groups of patients were higher than before treatment,and fixed dazzle group were higher than those in the triple combination group(all P<0.01).There was no statistically significant difference in the total incidence of adverse reactions between triple combination group and fixed dazzle group(6.67%vs.10.00%,χ^(2)=0.436,P=0.509).Conclusion Modified Shouzhong Dingxuan decoction can improve the cerebral hemodynamics of patients with PCIV,it can improve efficacy,and it has good safety,which is worthy of promotion and application.
