复元活血汤联合乳果糖治疗骨质疏松性椎体压缩骨折术后便秘临床疗效观察

Clinical Observation of Fuyuan Huoxue Decoction Combined with Lactulose in Treating Constipation after Osteoporotic Thoracolumbar Fracture

目的:探讨复元活血汤治疗骨质疏松性椎体压缩性骨折(OVCF)术后便秘的临床疗效。方法:选取2020年1月-2021年2月上海中医药大学附属岳阳中西结合医院收治的OVCF术后便秘患者62例,随机数字表法分为对照组和观察组,各31例。对照组采用乳果糖治疗,观察组在对照组基础上加用复元活血汤治疗,均连续治疗7 d。比较两组患者临床疗效和术后大便性状、便秘症状、腹围及不良反应发生情况。结果:观察组临床治疗总有效率明显高于对照组(P<0.05);治疗后观察组Bristol大便分型量表(BSFC)评分明显高于对照组,观察组症状自评量表(PAC-SYM)和腹围明显低于对照组(P<0.05);对照组不良反应发生率稍高于观察组,但差异无统计学意义(P>0.05)。结论:复元活血汤治疗OVCF术后便秘疗效确切,可有效改善大便性状及便秘症状,缩小腹围,提升骨折术后患者的生活质量,且安全性高,值得推广。

Objective To explore the clinical effect of Fuyuan Huoxue Decoction in the treatment of constipation after osteoporotic vertebral compression fracture(OVCF)surgery.Methods A total of 62patients with postoperative constipation after OVCF admitted to the Orthopedics Center of Yueyang Hospital of Integrated Traditional Chinese and Western Medicine affiliated to Shanghai University of Traditional Chinese Medicine(January 2020-February 2021)were selected and divided into two groups according to a random number table.The control group(n=31)received lactulose treatment,and the observation group was treated with Fuyuan Huoxue Decoction on the basis of the control group.Both groups were treated for 7consecutive days.The clinical efficacy and stool characteristics,constipation symptoms,abdominal circumference and adverse reactions were compared between the two groups after treatment.Results The total effective rate of clinical treatment in the observation group was significantly higher than that of the control group(P<0.05).The Bristol Stool Form Scale(BSFC)score of the observation group was significantly higher than that of the control group,and the Patient Assessment of Constipation Symptoms(PAC-SYM)and abdominal circumference in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the control group was slightly higher than that in the observation group,but the difference was not statistically significant(P>0.05).Conclusion Fuyuan Huoxue Decoction has definite efficacy in the treatment of constipation after OVCF,which can effectively improve stool characteristics and constipation symptoms,reduce abdominal circumference,improve the quality of life of patients after fracture surgery,It has high safety and is worthy of promotion.
