沙参麦冬汤加减联合穴位点刺治疗鼻咽癌放疗后口干症对中医症候积分及VAS评分的影响

Effects of modified Shashen Maidong decoction combined with acupoint pricking in the treatment of xerostomia after radiotherapy for nasopharyngeal carcinoma on TCM symptoms scores and VAS score

目的:探讨沙参麦冬汤加减联合穴位点刺治疗鼻咽癌放疗后口干症对患者中医症候积分及疼痛视觉模拟评分(VAS)评分的影响。方法:将我院从2018年10月~2021年10月就诊的60例鼻咽癌放疗后口干症患者作为研究对象,根据随机数字表法分为试验组和对照组,每组各30例。两组均接受鼻咽癌根治性放疗,在此基础上,试验组予以沙参麦冬汤加减口服联合金津、玉液点刺放血,对照组予以康复新液口服联合假针刺治疗,两组治疗均以1月为1疗程,总共治疗3个疗程,观察记录患者治疗前、治疗后和治疗后3个月的口干燥评分、唾液腺分泌指数(EF)、患者喉咙疼痛VAS评分以及患者治疗前后的中医症候评分。结果:治疗后、治疗后3个月,试验组的口干燥评分、VAS评分均低于对照组(P<0.05),EF值均高于对照组(P<0.05);治疗后,试验组中医证候评分均低于对照组(P<0.05)。结论:沙参麦冬汤加减联合穴位点刺可明显缓解鼻咽癌患者放疗后的口干症状,同时可降低中医症候积分及VAS评分,促进唾液腺分泌功能恢复,值得临床推广。

Objective:To explore the effects of modified Shashen Maidong decoction combined with acupoint pricking on scores of TCM symptoms and score of visual analogue scale(VAS)in patients with xerostomia after radiotherapy for nasopharyngeal carcinoma.Methods:60patients with xerostomia after radiotherapy for nasopharyngeal carcinoma admitted to the hospital from October 2018to October 2021were selected as the research subjects,and were divided into experimental group and control group with 30cases in each group according to the random number table method.Both groups received radical radiotherapy for nasopharyngeal carcinoma,and on this basis,experimental group took modified Shashen Maidong decoction and performed Jinjin and Yuye pricking and and bloodletting,while control group was given Kangfuxin solution orally combined with sham acupuncture.Both groups were treated for 3courses with 1month as a course of treatment.The mouth dryness score,salivary gland secretion index(EF)and throat pain VAS score before and after treatment and at 3months after treatment and scores of TCM symptoms before and after treatment were observed and recorded.Results:After treatment and at 3months after treatment,the mouth dryness score and VAS score of experimental group were lower than those of control group(P<0.05),and the EF value was higher than that of control group(P<0.05).After treatment,the scores of TCM syndromes were lower of experimental group than those of control group(P<0.05).Conclusion:Modified Shashen Maidong decoction combined with acupoint pricking therapy can significantly relieve the xerostomia symptoms in patients with nasopharyngeal carcinoma after radiotherapy,and can reduce the TCM symptoms scores and VAS score,and promote the recovery of salivary gland secretion function,which is worthy of clinical promotion.
