老年股骨颈骨折应用全髋和半髋关节置换术治疗的疗效比较

Comparison of the curative effect of total and hemi-hip arthroplasty in the treatment of femoral neck fractures in the elderly

目的对比全髋和半髋关节置换术治疗老年股骨颈骨折的疗效。方法50例老年股骨颈骨折患者,以入院单双日分为观察组(单日)和对照组(双日),每组25例。观察组采用全髋关节置换术治疗,对照组采用半髋关节置换术治疗。比较两组患者的康复效果,关节功能。结果治疗后,观察组优14例(56.00%)、良8例(32.00%)、可2例(8.00%)、差1例(4.00%),对照组优8例(32.00%)、良5例(20.00%)、可5例(20.00%)、差7例(28.00%)。观察组的康复效果优于对照组,差异具有统计学意义(Z=2.4670,P=0.0136<0.05)。治疗前,两组患者的Harris评分比较,差异无统计学意义(P>0.05);治疗后,观察组的Harris评分(89.78±6.82)分高于对照组的(80.23±8.33)分,差异具有统计学意义(P<0.05)。结论老年股骨颈骨折患者采用全髋关节置换术的效果明显优于半髋关节置换术,可以提升临床疗效并改善患者的关节功能,值得推荐。

Objective To compare the curative effect of total and hemi-hip arthroplasty in the treatment of femoral neck fractures in the elderly.Methods A total of 50 elderly patients with femoral neck fracture were divided into observation group(odd-numbered day)and control group(even-numbered day)according to the odd-and even-numbered day of admission,with 25 cases in each group.The observation group was treated with total hip arthroplasty,and the control group was treated with semi-hip arthroplasty.The rehabilitation effect and joint function of the two groups were compared.Results After treatment,14 cases(56.00%)were excellent,8 cases(32.00%)were good,2 cases(8.00%)were acceptable,and 1 case(4.00%)was poor in the observation group;8 cases(32.00%)were excellent,5 cases(20.00%)were good,5 cases(20.00%)were acceptable,and 7 cases(28.00%)were poor in the control group.The rehabilitation effect of the observation group was better than that of the control group,and the difference was statistically significant(Z=2.4670,P=0.0136<0.05).Before treatment,there was no statistically significant difference in Harris scores between the two groups(P>0.05).After treatment,the Harris score of(89.78±6.82)points in the observation group was higher than that of(80.23±8.33)points in the control group,and the difference was statistically significant(P<0.05).Conclusion The effect of total hip arthroplasty for elderly patients with femoral neck fractures is significantly better than that of hemihip arthroplasty,which can improve the clinical efficacy and improve the joint function of patients,and it is worth recommending.
