全方位干预对无痛胃肠镜检查患者应激反应、不良事件的影响

Effects of comprehensive intervention on stress response and adverse events in patients undergoing painless gastroenteroscopy

目的研究全方位干预措施对无痛胃肠镜检查患者应激反应、血流动力学、不良事件的影响。方法选取2021年1~12月行无痛胃肠镜检查术患者1800例为研究对象,以抽签法随机分为观察组与对照组,2组均接受常规护理,观察组另外采取术前、术中、术后的全方位干预,比较2组患者应激指标、血流动力学、不良反应及护理满意度。结果检查后观察组患者MDA(6.21±0.19)μmol/L水平显著低于对照组,SOD(90.77±11.54)U/L水平显著高于对照组,2组比较差异有统计学意义(P<0.05)。检查后观察组患者收缩压(132.32±5.12)mm Hg、舒张压(78.54±3.12)mm Hg、心率(79.43±0.62)次/min等指标低于对照组,2组比较差异有统计学意义(P<0.05),观察组并发心动过缓18例,一次低血压10例,一次血氧饱和度下降5例,呃逆3例,恶心呕吐2例,寒战2例,苏醒延迟2例,过敏2例,躁郁3例,并发症总发生率为5.22%,显著低于对照组的8.67%,组间比较差异有统计学意义(P<0.05),观察组总满意度为97.67%,显著高于对照组的84.78%,2组差异有统计学意义(P<0.05)。结论全面护理可保障无痛胃肠镜患者的安全,降低患者检查后的应激反应状况,减少并发症发生率,提升护理满意度,可广泛推广应用。

Objective To investigate the effects of comprehensive intervention on stress response,hemodynamics and adverse events in patients undergoing painless gastroenteroscopy.Methods A total of 1800 patients undergoing painless gastroenteroscopy in our hospital from January 2021 to December 2021 were selected as the research objects and they were randomly allocated to the observation group and the control group by lottery method.Routine nursing was given to all patients,and those in the observation group were additionally given to preoperative,intraoperative and postoperative comprehensive intervention.The stress index,hemodynamics,adverse reactions and nursing satisfaction were compared between the two groups.Results After intervention,the malondialdehyde(MDA)level([6.21±0.19]μmol/L)in the observation group was significantly lower than that in control group,while Superoxide dismutase(SOD)level([90.77±11.54]U/L)was significantly higher(P<0.05).The systolic blood pressure([132.32±5.12]mmHg),diastolic blood pressure([78.54±3.12]mmHg),and heart rate([79.43±0.62]beats/min)in the observation group were significantly lower than those in the control group(P<0.05).There were 18 cases of bradycardia,10 cases of hypotension,5 cases of blood oxygen saturation drop,3 cases of hiccup,3 cases of manic depression,2 cases of nausea and vomiting,2 cases of chills,2 cases of delayed awakening and 2 cases of allergy in the observation group.The incidence of complications in the observation group was significantly lower than that of control group(5.22%vs 8.67%,P<0.05),while the satisfaction rate was significantly higher(97.67%vs 84.78%,P<0.05).Conclusion Comprehensive nursing can ensure the safety of patients with painless gastroenteroscopy,reduce the stress response and the incidence of complications after examination,and improve nursing satisfaction,which can be widely applied.
