产ESBLs的肺炎克雷伯菌在老年患者中的随机多态DNA分型研究

摘要 目的 了解我院老年患者中产 ESBLs的肺炎克雷伯菌的耐药特点。方法 对 2 4 5名住院的 60岁以上患者感染性标本分离培养后 ,用全自动微生物鉴定及药敏系统 (VITE- AMS)进行鉴定和药敏 ,然后对产 ESBL s株进行 RAPD分型。结果 分离出 88株肺炎克雷伯菌 (35.5% ) ,其中产 ESBLs的 2 0株 (2 2 .7% ) ,用 RAPD技术可分为 1 1型。结论 产 ESBLs的肺炎克雷伯菌是引起老年患者感染的病原菌之一 ,对其型别的检测有利于流行病学的调查和控制 ESBLs的传播和流行 ,使老年患者的院内感染性疾病得以控制。 Objectives To determine current resistance of ESBL producing K.pneumoniae in old patients.Methods 245 specimens from over 60 years old patients were collected and cultured, and all of the isolates were identified as well as subjected to drug susceptibility test by VITEK AMS. The ESBL producing strains were typed by RAPD. Results Total of 88 isolates were K.pneumoniae positive, the isolating rate was 35.5%, and 20 of them were ESBL positive (22.7%). By RAPD, all of ESBL producing strains could be divided into Ⅱ types. Conclusions ESBL producing K.pneumoniae is one of the primary pathogens that resulted in infection diseases in old patients. Typing of them is a useful means for epidemiological investigation and for transmition control of ESBLs. Thereby, hospital acquired infection of old patients will be controlled well.
