起源于冠状静脉窦憩室口部旁道消融一例

摘要 患者女性,34岁,因心悸、胸闷1个月,加重再发1h入院.消融术中冠状窦(CS)5/6—CS 7/8电极前传V波早,心室起搏时逆传亦是CS 5/6—CS 7/8电极A波早,考虑为左后间隔旁道,消融失败;后拟至对应的外膜面(冠状静脉窦内)标测,发现在冠状窦内有一往外凸出的条索状结构,造影后显示为憩室,在憩室口部标测到非常理想靶点(AV融合,逆传亦提前),消融几秒后预激波消失,逆传VA亦分离,达到即刻成功,无手术并发证发生.
