一次高空落物事件的调查及启示

INVESTIGATION AND REVELATION OF A DROPPED OBJECT INCIDENT

高空落物事件多发生在钻修井的钻台面上,常导致钻台作业人员的伤亡,故钻台面上的设备在设计、制造及使用过程中,需考虑落物的风险。对1起钻台面高空落物事件进行了调查,发现了该高空设备在设计、制造、使用过程中的缺陷。提出提高风险意识及准确的风险识别是减少事故发生的两个重要基础,完善的落物管理系统需覆盖设备的设计、制造、使用全生命周期,从而避免落物事故的发生。

The dropped object incident mostly happens on the drilling floor,which often leads to the casualties of drilling floor operators.Therefore,the risk of dropped objects needs to be considered in the design,manufacture and use of the equipment on the drilling floor.In this paper,an investigation was conducted on a dropped object incident on the drilling floor,and the defects in the design,manufacture and use of the equipment were found.It is pointed out that improving risk awareness and accurate risk identification are two important bases for reducing accidents.A well-developed dropped object management system should cover the whole life cycle of equipment design,manufacture and use so as to avoid dropped object accidents.
