临床路径法和PBL教学法在心儿科住培中的应用

摘要 住院医师规范化培训是医学生毕业后教育的重要组成部分。心血管儿科(简称心儿科)作为儿科住培基地中的亚专科,专业性强和特色优势明显,在住院患者中有较高的临床路径入组率。但心儿科病种复杂多样,住培生学习难度较大,如何在短期内实现培养目的,已成为住培基地规范化教学的当务之急。广东省人民医院是广东省内最早开展住院医师和全科医师规范化培训的培训基地之一,师资力量雄厚,带教老师具有多年带教的经验,知识储备丰富。带教老师可结合科室特色,应用临床路径法结合以问题为导向的教学法(problem-based learning,PBL),根据教学大纲从理论到实际、以问题为核心、由浅到深进行带教,发挥学员的主观能动性,提高学习的主动性和积极性。带教后,学员解决问题、主动学习、团队沟通协作等能力得到提高,证实了临床路径法结合PBL教学法在心儿科住培中的良好应用价值。 The standardized training of residents is an important part of the post-graduation education of medical students.As a sub-specialty in pediatric residential training base,cardiovascular pediatrics(referred to as cardiac pediatrics)has strong professionalism and obvious characteristics,and has a high clinical pathway enrollment rate in inpatients.However,the types of cardiovascular diseases are complicated and diverse,and it is difficult for residential students to learn.How to achieve the purpose of training in a short period of time has become the urgent task of standardized teaching in residential training bases.Guangdong Provincial People's Hospital is one of the first training bases in Guangdong Province to carry out standardized training for resident doctors and general practitioners.It has a strong faculty and teachers with many years of experience in teaching and rich knowledge reserves.Teachers can combine the characteristics of the department,apply the clinical pathway method and problem-based learning(PBL),and conduct the teaching from theory to practice,from shallow to deep according to the teaching syllabus,taking the problem as the core,so as to exert the students'subjective initiative and improve their learning initiative and enthusiasm.After teaching,the students'ability of problem-solving,active learning,team communication and cooperation was improved,which confirmed the good application value of clinical pathway method combined with PBL teaching model in pediatric residential training.
