摘要 目的研究呼吸危重症监护病区医学教学工作开展中使用迷你临床演练评估(mini clinical exercise evaluation,Mini-CEX)结合操作技能直接观察法(direct observation of operational skills,DOPS)评价体系的临床效果。方法于2020年5月—2021年10月选入40名呼吸危重症监护病区实习生作为研究对象,按单双号分组方法等分为对照组20名和研究组20名,分别使用常规医学教学法、基于Mini-CEX结合DOPS评价体系的医学教学法,将两组教学效果实施比较分析。结果比较两组教学效果,研究组在“提升学习兴趣、提升问题解决能力、提升表达能力、提升临床思维能力、总分”五个方面的评分结果均明显高于对照组,差异有统计学意义(P<0.05);比较两组教学满意度,研究组95.00%(19/20)明显高于对照组70.00%(14/20),差异有统计学意义(P<0.05);比较两组考核成绩,研究组出科时在“理论知识、专业技能、病历书写、总分”四个方面的评分结果明显高于对照组,差异有统计学意义(P<0.05);比较研究组实习生入科时、出科时Mini-CEX评分、DOPS评分变化,显示其各项分值评定结果均有大幅提升,差异有统计学意义(P<0.05)。结论通过使用Mini-CEX结合DOPS评价体系开展呼吸危重症监护病区医学教学工作,有助于教学效果,整体满意度较高。 Objective To study the clinical effect of mini clinical exercise evaluation(Mini-CEX)combined with direct observation of operational skills(DOPS)evaluation system in medical teaching in respiratory intensive care unit.Methods A total of 40 interns in respiratory intensive care ward were selected as the research objects from May 2020 to October 2021.They were divided into control group(20 students)and analysis group(20 students)according to odd-even grouping methods,respectively using conventional medical teaching methods and medical teaching methods based on Mini-CEX combined with DOPS evaluation system.The teaching effect of the two groups was analyzed.Results Compared the teaching effects of the two groups,the scores of the analysis group in five aspects of"improving learning interest,improving problem solving ability,improving expression ability,improving clinical thinking ability and total score"were significantly higher than those of the control group,with statistical significance(P<0.05).The teaching satisfaction of the analysis group was 95.00%(19/20)higher than that of the control group 70.00%(14/20),and the differences were statistically significant(P<0.05).Compared the scores of the two groups,the scores of the analysis group in"theoretical knowledge,professional skills,medical record writing and total score"were significantly higher than those of the control group,and the differences were statistically significant(P<0.05).The changes of Mini-CEX score and DOPS score of interns in the analysis group were compared when they entered the program and when they left the program,which showed that the evaluation results of each score had been significantly improved,and the differences were statistically significant(P<0.05).Conclusion The application of Mini-CEX combined with DOPS evaluation system to carry out medical teaching in respiratory intensive care unit is conducive to the teaching effect,with high overall satisfaction.
