大剂量与小剂量左甲状腺素钠片治疗妊娠合并甲状腺功能减退症的疗效研究

Curative Effect Study on High-Dose and Low-Dose Levothyroxine Sodium Tablets for Pregnancy with Hypothyroidism

目的 探讨在妊娠合并甲状腺功能减退症治疗中应用大剂量与小剂量左甲状腺素钠片的效果。方法 选取本院2021年7月-2022年7月诊治的76例妊娠合并甲状腺功能减退症患者,随机分为两组,每组38例。参照组应用小剂量左甲状腺素钠片治疗,实验组应用大剂量左甲状腺素钠片治疗,分析治疗前后两组甲状腺激素水平、血脂、叶酸情况、妊娠结局。结果 治疗后,实验组促甲状腺素、游离三碘甲状腺原氨酸、游离四碘甲状腺原氨酸低于参照组(P<0.05);治疗后,实验组甘油三酯、总胆固醇、低密度脂蛋白胆固醇低于参照组(P<0.05);实验组高密度脂蛋白胆固、叶酸高于参照组(P<0.05)。结论 在妊娠合并甲状腺功能减退症治疗过程中应用大剂量左甲状腺素钠片,可显著改善甲状腺功能,促使血脂下降,提高体内叶酸的水平,值得普及和推广。

Objective To explore effect of high-dose and low-dose levothyroxine sodium tablets in treatment of pregnancy with hypothyroidism.Methods The paper chose 76 patients with gestational hypothyroidism diagnosed and treated in our hospital from July 2021 to July 2022,and divided them into two groups randomly,with 38 cases in each group.Reference group was treated with low-dose levothyroxine sodium tablets,and experimental group with high-dose levothyroxine sodium tablets.Levels of thyroid hormone,blood lipids,folic acid and pregnancy outcomes between two groups were analyzed before and after treatment.Results After treatment,levels of thyrotropin,free triiodothyronine and free tetraiodothyronine in experimental group were lower than control group(P<0.05).After treatment,levels of triglycerides,total cholesterol andlow-density lipoprotein cholesterol in experimental group were lower than control group(P<0.05).High-density lipoprotein cholesterol and folate in experimental group was higher than control group(P<0.05).Conclusion High-dose levothyroxine sodium tablets can significantly improve thyroid function,promote decrease of blood lipids and increase level of body folic acid in treatment of pregnancy complicated with hypothyroidism,which is worthy of promotion.
