动态心电图应用在冠心病心律失常患者诊断中的临床效果

Clinical Effect of Dynamic Electrocardiogram Application in Diagnosis of Coronary Heart Disease and Arrhythmia Patients

目的 对冠心病心律失常患者进行动态心电图检查的诊断效能展开分析。方法 将2019年1月-2022年1月在本院进行心电图检查的冠心病心律失常患者50例纳入观察主体,分别进行普通心电图检查和动态心电图检查,对两种检查的诊断效果进行比较。结果 患者通过动态心电图检查在心肌缺血、心律失常方面的阳性检出率均高于普通心电图检查,组间数据相对比,差异有统计学意义(P<0.05);动态心电图检查在房性早搏二、三联律,室性早搏二、三联律、短阵室上速、房性早搏成对、室性早搏成对等方面病情的检出率与普通心电图检查相比较,差异有统计学意义(P<0.05),但是在心房颤动,束支传导阻滞,短阵室速,房室传导阻滞等方面的检出率比较,差异无统计学意义(P>0.05);动态心电图检查的准确率、灵敏度、特异度优于普通心电图检查,两组组间数据相比较,差异有统计学意义(P<0.05)。结论 相比较普通心电图检查而言,动态心电图检查可提升心律失常、心肌缺血等阳性检出率,临床诊断价值更高,值得推荐。

Objective To analyze diagnostic effect of dynamic electrocardiogram examination for patients with coronary heart disease and arrhythmia.Methods The paper chose 50 patients with coronary heart disease and arrhythmia with electrocardiogram examination in our hospital from January 2019 to January 2022 as observation objects,with regular electrocardiogram examination and dynamic electrocardiogram examination respectively.Diagnostic effect between two examinations was compared.Results Positive detection rate of myocardial ischemia and arrhythmia of patients with dynamic electrocardiogram examination was higher than regular electrocardiogram examination,difference between groups was significant(P<0.05).Detection rate comparison of dynamic electrocardiogram examination in aspects of biphasic and triadic rhythm of atrial premature beats,biphasic and triadic rhythm of ventricular premature beats,short wave supraventricular tachycardia,paired atrial premature beats and equal ventricular premature beats showed statistical significance compared with ordinary electrocardiogram examination,difference between groups was significant(P<0.05).However,detection rate comparison of atrial fibrillation,bundle branch block,short wave ventricular tachycardia and atrioventricular block showed no statistically significant difference between two groups(P>0.05).Accuracy,sensitivity and specificity of dynamic electrocardiogram examination was 96.00%,92.00%and 86.00%respectively,that in ordinary electrocardiogram examination was 76.00%,70.00%and 68.00%respectively,difference between two groups was statistically significant(P<0.05).Conclusion Dynamic electrocardiogram examination can improve positive detection rate of arrhythmia,myocardial ischemia,etc,with higher clinical diagnostic value than than ordinary electrocardiogram examination,which is worth recommending.
