人工桡骨小头置换术治疗肘关节恐怖三联征的临床有效性研究

Clinical Effect Research of Artificial Radial Head Replacement Surgery in Treatment of Cubital Joint Phobia Triad

目的 分析针对肘关节恐怖三联征患者,应用人工桡骨小头置换的临床疗效。方法 抽取2021年5月-2022年5月就诊本院的肘关节恐怖三联征患者共计40例,计算机1:1随机数列均分组别,参比组(切开复位内固定,n=20),分析组(人工桡骨小头置换术治疗,n=20)。比较两组手术指标、肘关节活动度及评分及并发症发生率。结果 分析组手术时间及住院时间显著短于参比组,术后1d疼痛评分显著低于参比组,数据比较有意义(P<0.05);术后3个月时,分析组肘关节旋前角度与选后角度均显著高于参比组,数据比较有意义(P<0.05);但关节曲角度数据两组比较无意义(P>0.05);分析组术后6个月时,关节疼痛度低于参比组,关节活动功能由于参比组,数据比较有意义(P<0.05);关节稳定性与关节日常活动能力数据比较无意义(P>0.05);参比组并发症发生率显著高于分析组,数据比较有意义(χ^(2)=4.329,P=0.037)。结论 在肘关节恐怖三联征治疗中,应用人工桡骨小头置换术能够利于手术指标改善,改善肘关节功能,并降低并发症发生率,可推广。

Objective To analyze clinical effect of artificial radial head replacement for patients with cubital joint phobia triad.Methods The paper chose 40 patients with cubital joint phobia triad who visited our hospital from May 2021 to May 2022 were randomly divided them into two groups:reference group(open reduction and internal fixation,n=20)and analysis group(artificial radial head replacement surgery,n=20).was compared surgical indicators,elbow joint range of motion and scores and incidence of complications between two groups.Results Surgery time and length of stay in analysis group was significantly shorter than reference group,pain scores on the 1st day after surgery were significantly lower than reference group,indicating significant data difference(P<0.05).At 3 months after surgery,elbow joint pronation angle and selected posterior angle in analysis group was significantly higher than reference group,data were statistically significant(P<0.05).However,there was no significant difference between two groups in terms of joint curvature angle data(P>0.05).At 6 months after surgery,the degree of joint pain in analysis group was lower than reference group,joint activity function was more significant than reference group(P<0.05).Comparison of joint stability and daily activity ability data was not significant(P>0.05).Incidence of complications in reference group was significantly higher than analysis group,data were significant(χ^(2)=4.329,P=0.037).Conclusion In treatment of elbow joint phobia triad,artificial radial head replacement surgery can improve surgical indicators,elbow joint function and reduce incidence of complications,which can be promoted.
