前庭功能联合眼震视图检查在鉴别眩晕疾病中的诊断价值研究

Diagnostic Value Research of Vestibular Function Combined with Videonystagmusgraphy in Distinguishing Vertigo Diseases

目的 探讨前庭功能联合眼震视图检查鉴别眩晕疾病的诊断价值。方法 选取2021年12月-2022年6月本院收治的85例眩晕疾病患者,统计两组患者前庭功能、眼震视图诊断结果,参照临床诊断,分析前庭功能、眼震视图诊断效果,并分析两者联合诊断效果。结果 前庭功能联合眼震视图检查对外周性眩晕鉴别诊断灵敏度、特异度和准确率,显著高于单独诊断(P<0.05)。参照临床诊断结果:前庭功能检查对中枢性眩晕鉴别诊断灵敏度、特异度和准确率,与眼震视图诊断无显著差异,无统计学意义(P>0.05);前庭功能联合眼震视图检查对中枢性眩晕鉴别诊断灵敏度、特异度和准确率,显著高于单独诊断(P<0.05)。结论 前庭功能、眼震视图检查均可为眩晕疾病鉴别提供参考资料,但是存在较高的漏诊、误诊风险,两者联合诊断,可有效提升眩晕疾病鉴别的特异度和敏感性,显著提升了诊断准确性,临床诊断效果较为理想,可为鉴别眩晕疾病提供有效的数据支持,值得推广借鉴。

Objective To explore diagnostic value of combining vestibular function with videonystagmusgraphy in distinguishing vertigo diseases.Methods The paper chose 85 patients with vertigo admitted to our hospital from December 2021 to June 2022,carried on statistical analysis of diagnosis effect of vestibular function and videonystagmusgraphy of both groups,and combined diagnostic effect of two was analyzed.Results Based on clinical diagnostic results:sensitivity,specificity,and accuracy of combined examination of vestibular function and videonystagmusgraphy in differential diagnosis of peripheral vertigo was significantly higher than individual diagnosis(P 0.05).Referring to clinical diagnostic results,sensitivity,specificity and accuracy of vestibular function examination in differential diagnosis of central vertigo was not significantly different from diagnosis of videonystagmusgraphy,with no statistical significance(P 0.05);sensitivity,specificity,and accuracy of combined examination of vestibular function and videonystagmusgraphy in differential diagnosis of central vertigo was significantly higher than individual diagnosis(P 0.05).Conclusion Vestibular function and videonystagmusgraphy examination can provide reference materials for differential diagnosis of vertigo diseases,but there is high risk of missed diagnosis and misdiagnosis.Combination of them can improve specificity and sensitivity of differential diagnosis of vertigo diseases effectively,improve diagnostic accuracy significantly with relatively ideal clinical diagnostic effect,with effective data support for differential diagnosis of vertigo diseases,which is worthy of promotion and reference.
