彩色多普勒超声在乳腺良恶性肿瘤鉴别诊断中的价值 被引量:2

摘要 目的 探讨临床鉴别诊断乳腺良恶性肿瘤疾病方案选择彩色多普勒超声技术的准确率及应用价值。方法 选择2017年6月-2022年6月于本院接受诊断治疗的乳腺肿瘤患者110例作为此次研究对象,均接受彩色多普勒超声技术诊断,根据病理诊断结果作为最终结果,计算比较彩色多普勒超声技术的检出率、灵敏度与特异性,比较分析良恶性乳腺肿瘤患者血流动力学指标(阻力指数、血流收缩期峰值流速)及血流分级情况,归纳总结乳腺良性肿瘤患者与恶性肿瘤患者超声影像表现特点。结果 110例乳腺肿瘤患者通过病理最终诊断显示,良性肿瘤患者共50例,占比45.45%,包括:乳腺纤维瘤22例,乳腺脂肪瘤14例,乳腺增生8例,其他类型乳腺良性肿瘤6例。恶性肿瘤患者共60例,占比54.55%。对比彩色多普勒超声技术检出率、灵敏度、特异度与病理诊断结果无统计学差异(P>0.05)。良性乳腺肿瘤患者经检测,阻力指数、血流收缩期峰值流速明显低于恶性乳腺肿瘤患者(P<0.05),良性乳腺肿瘤患者血流等级0级占比高于恶性肿瘤,Ⅱ级、Ⅲ级占比低于恶性肿瘤(P<0.05)。另外,良性乳腺肿瘤患者彩超影像病灶呈圆形或椭圆形,边界清晰,回声均匀,恶性乳腺肿瘤患者彩超影像病灶形态不规则,边界模糊,有浸润征象。结论 彩色超声多普勒技术用于临床鉴别诊断乳腺良恶性肿瘤具有灵敏度高、准确率高等优点,且操作便捷,影像清晰,可为疾病制定进一步治疗方案提供保障,值得运用推广。 Objective To explore accuracy and application value of color Doppler ultrasound technology in clinical differential diagnosis of benign and malignant breast tumor diseases.Methods The paper chose 110 breast tumor patients diagnosed and treated in our hospital from June 2017 to June 2022 as study objects.Based on color Doppler ultrasound pathological diagnosis results,detection rate,sensitivity,and specificity of color Doppler ultrasound technology was calculated and compared.Hemodynamic indicators(resistance index,peak systolic flow velocity) and blood flow grading of patients with benign and malignant breast tumors were compared and analyzed,ultrasound imaging characteristics of patients with benign and malignant breast tumors were summarized.Results Final pathological diagnosis of 110 breast tumor patients showed,there were 50 cases of benign tumors,accounting for 45.45%,including 22 cases of breast fibroma,14 cases of breast lipoma,8 cases of breast hyperplasia,and 6 cases of other types of breast benign tumors.There were 60 patients with malignant tumors,accounting for 54.55%,with no statistically significant difference in detection rate,sensitivity,specificity,and pathological diagnosis results between color Doppler ultrasound techniques(P0.05).After detection,patients with benign breast tumors showed significantly lower resistance index and peak systolic blood flow velocity than patients with malignant breast tumors(P0.05).Proportion of blood flow grade O of patients with benign breast tumors was higher than malignant tumors,while proportion of blood flow grade Ⅱ and Ⅲ was lower than malignant tumors(P0.05).In addition,lesion of patients with benign breast tumors showed circular or elliptical in color Doppler imaging,with clear boundaries and uniform echoes.Lesion morphology of patients with malignant breast tumors wass irregular,with blurred boundaries and signs of infiltration.Conclusion Color Doppler ultrasound technology has advantages of high sensitivity,accurate results,convenient operation,clear imaging in clinical differential diagnosis of benign and malignant breast tumors,can provide guarantees for further development of disease treatment scheme,which is worthy of application and promotion.
