紫杉醇联合奈达铂放化疗治疗中晚期食管癌的临床分析

Clinical Analysis of Paclitaxel Combined with Nedaplatin in Treatment of Advanced Esophageal Cancer

目的 观察调强放射治疗联合紫杉醇及奈达铂化疗对中晚期食管癌的治疗效果。方法 选取2019年1月-2022年1月本院收治的70例中晚期食管癌患者,KPS评分不低于70分,随机分成治疗组和对照组,每组各35例,所有患者均采用调强放射治疗,治疗组在调强放疗的第1d同时静脉滴注紫杉醇150mg/m~2、奈达铂64mg/m~2,每3周为1周期,对照组采用单纯调强放射治疗,治疗组患者接受调强放射治疗期间同步化疗2个周期,放疗结束后再巩固化疗2~4个周期。结果 治疗组有效率明显优于对照组,差异具有统计学意义(P<0.05);治疗组不良反应发生率略高于对照组,差异无统计学意义(P>0.05)。结论 紫杉醇联合奈达铂放化疗治疗中晚期食管癌能显著提高治疗效果,而不良反应未见明显加重,患者基本能够耐受该方案,因此该治疗方法在临床上值得进一步推广使用。

Objective To observe curative effect and side effect of intensity modulated radiotherapy(IMRT)combined with paclitaxel and nedaplatin chemotherapy in treatment of advanced esophageal cancer.Methods The paper chose 70 patients with advanced esophageal cancer admitted to our hospital from January 2019 to January 2022,with KPS scores no less than 70,and divided them into treatment group and control group randomly,with 35 cases in each group.All patients were treated with intensity modulated radiation therapy,treatment group was treated with intravenous drip of paclitaxel 150mg/m2 and nedaplatin 64mg/m2 on the first day of intensity modulated radiation therapy at the same time,with treat course of every three weeks.Control group was treated with intensity modulated radiation therapy only,treatment group with two cycles of synchronous chemotherapy during intensity modulated radiation therapy period,and 2-4 cycles of consolidated chemotherapy after the radiotherapy.Result:Effective rate of treatment group was significantly better than control group,with statistically significant difference(P<0.05).Incidence of adverse reactions in treatment group was slightly higher than control group,difference was not statistically significant(P>0.05).Conclusion Paclitaxel combined with nedaplatin radiotherapy and chemotherapy can improve treatment effect of advanced esophageal cancer significantly,with less aggravated adverse reactions.Patients can basically tolerate the scheme,which is worthy of further promotion and application in clinical practice.
