急性下肢深静脉血栓形成后持续压力治疗的疗效分析 被引量:1

摘要 目的探讨在抗凝治疗基础上持续弹力绷带压迫治疗对急性下肢深静脉血栓形成(ALEDVT)的临床效果。方法收集2019年8月至2021年5月于西安交通大学第一附属医院就诊的80例ALEDVT住院患者的临床资料,按照患者治疗方法的不同将其分为对照组(n=40,采用单纯抗凝治疗)和联合组(n=40,采用抗凝联合弹力压迫治疗)。比较两组患者治疗后的患肢腿围情况、临床症状改善情况、血管再通情况、住院时间。结果治疗3 d后,联合组患者患肢大腿、小腿腿围及其腿围差均明显小于对照组患者,差异均有统计学意义(P﹤0.01)。治疗3 d后,两组患者患肢疼痛症状均明显改善,整体缓解率为93.75%(75/80),其中,联合组患者缓解率为100%(40/40),明显高于对照组患者的87.50%(35/40),差异有统计学意义(P﹤0.01)。治疗1周后,两组患者整体再通率为87.50%(70/80),其中,联合组患者再通率为100%(40/40),明显高于对照组患者的75.00%(30/40),差异有统计学意义(P﹤0.01)。联合组患者住院天数明显短于对照组患者,差异有统计学意义(P﹤0.01)。结论ALEDVT患者在抗凝药物治疗的基础上加用弹力绷带持续加压可有效改善其临床症状,快速消肿,提高闭塞血管的再通率,值得推广应用。 Objective To investigate the clinical efficacy of continuous elastic bandage pressure therapy based on anticoagulation treatment in acute lower extremity deep venous thrombosis(ALEDVT).Method Clinical data of 80 ALEDVT patients treated in the First Affiliated Hospital of Xi'an Jiaotong University from August 2019 to May 2021 were selected,they were divided into control group(n=40,treated with single anticoagulation therapy)and combined group(n=40,treated with anticoagulation therapy+elastic bandage pressure therapy)according to different treatment methods.The extremity circumference,clinical symptom improvement,vascular recanalization and hospitalization time after treatment in two groups were compared.Result After 3 days of treatment,the thigh and calf circumference and difference of circumference in combined group were lower than those in the control group,the differences were statistically significant(P<0.01).After 3 days of treatment,the extremity pain improved significantly,with the overall remission rate of 93.75%(75/80),in which,the remission rate of combined group was 100%(40/40),higher than that in the control group of 87.50%(35/40),the difference was statistically significant(P<0.01).After 1 week of treatment,the overall vascular recanalization rate of patients in both groups was 87.50%(70/80),in which,the vascular recanalization rate of combined group was 100%(40/40),higher than that in the control group of 75.00%(30/40),the difference was statistically significant(P<0.01).The average hospitalization time in combined group was shorter than that in control group,the difference was statistically significant(P<0.01).Conclusion Continuous elastic bandage pressure therapy combined with anticoagulant therapy for ALEDVT can improve the clinical symptoms,reduce swelling,improve vascular recanalization rate,which is worthy of application.
