超声引导下内镜逆行性阑尾炎治疗术在小儿阑尾炎中的临床效果分析

Clinical Effectiveness of Ultrasound-guided Endoscopic Retrograde Appendicitis Treatment in Pediatric Appendicitis

目的:分析超声引导下内镜逆行性阑尾炎治疗术治疗小儿阑尾炎的临床效果及对插管成功率、复发率、手术指标的影响。方法:选取2019年1月—2021年5月禹州钧都医院收治的130例小儿阑尾炎患儿作为研究对象,依据治疗方法不同,将采用传统腹腔镜手术治疗的65例患儿列为对照组,将采用超声引导下内镜逆行性阑尾炎治疗的65例患儿列为观察组。对两组患儿治疗总有效率、插管成功率、手术情况、术后恢复情况、并发症发生率及复发率进行对比分析。结果:观察组的总有效率高于对照组,差异有统计学意义(χ^(2)=3.900,P<0.05);观察组的治疗中位时间、术后卧床时间、体温恢复正常时间、住院时间均低于对照组,差异有统计学意义(t=38.238、8.749、44.401、52.930,P<0.05);两组患儿并发症发生率比较,差异无统计学意义(χ^(2)=0.208,P>0.05);观察组的复发率明显低于对照组,差异有统计学意义(χ^(2)=3.876,P<0.05)。结论:超声引导下内镜逆行性阑尾炎治疗小儿阑尾炎具有良好的临床效果,能显著提高插管成功率,降低术后复发率。

Objective:To analyze the clinical effect of ultrasound-guided endoscopic retrograde appendicitis in the treatment of pediatric appendicitis and the effect on the success rate of intubation,recurrence rate,and surgical index.Methods:130 children with pediatric appendicitis admitted to the hospital from January 2019 to May 2021 were selected as the study population.According to the different treatment methods,65 children treated by conventional laparoscopic surgery were classified as the control group and 65 children treated by ultrasound-guided endoscopic retrograde appendicitis were classified as the observation group.The total efficiency of treatment,successful intubation rate,surgery,postoperative recovery,complication rate and recurrence rate of the two groups were compared and analyzed.Results:The overall effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(χ^(2)=3.900,P<0.05).The median time to treatment,time to postoperative bed rest,time to normalization of body temperature,and time to hospitalization were all lower in the observation group than in the control group,with statistically significant differences(t=38.238,8.749,44.401,52.930,P<0.05).There was no statistically significant difference in the incidence of complications between the two groups of patients(χ^(2)=0.208,P>0.05).The recurrence rate in the observation group was significantly lower than that in the control group,and the difference was statistically significant(χ^(2)=3.876,P<0.05).Conclusion:Ultrasound-guided endoscopic retrograde appendicitis for pediatric appendicitis has good clinical results and can significantly improve the success rate of intubation and reduce the recurrence rate after surgery.
