摘要
目的:探讨循证护理对学龄期特发性血小板减少性紫癜(ITP)患儿心理状态及治疗依从性的影响。方法:选取2018年6月—2020年4月郑州市第一人民医院就诊的88例ITP患儿,按照随机数表法将其分为对照组(44例)和观察组(44例)。对照组给予常规护理,观察组采用循证护理。对比两组患儿心理状态、治疗依从性、生活质量及家长护理满意度,持续干预3个月。结果:干预前两组患儿心理状态比较,差异无统计学意义(P>0.05);干预后观察组焦虑自评量表(SAS)评分为(40.32±5.75)分、抑郁自评量表(SDS)评分为(42.51±6.57)分,低于对照组的(45.54±6.07)分和(47.70±5.19)分,差异有统计学意义(t=4.203、4.112,P<0.05);观察组治疗依从性为95.45%,高于对照组的79.55%,差异有统计学意义(χ^(2)=5.061,P<0.05);干预前两组患儿生活质量评分比较,差异无统计学意义(P>0.05);干预后观察组患儿生活质量普适性核心评定量表(PedsQL4.0)中的生理维度评分为(88.94±4.63)分、社会维度评分为(91.12±4.29)分、情感维度评分为(85.49±5.10)分、角色维度评分为(83.32±5.11)分,高于对照组的(82.43±5.54)分、(83.86±5.47)分、(76.31±5.62)分、(76.44±5.20)分,差异有统计学意义(t=5.981、6.928、8.025、6.260,P<0.05);观察组家长护理满意度为95.45%,高于对照组的81.82%,差异有统计学意义(χ^(2)=54.062,P<0.05)。结论:循证护理能够改善学龄期ITP患儿心理状态,提升治疗依从性与家属护理满意度,改善患儿生活质量。
Objective:To investigate the impact of evidence-based care on psychological status and treatment compliance in school-aged children with idiopathic thrombocytopenic purpura(ITP).Methods:88 children with ITP in the hospital from June 2018 to April 2020 were selected and divided into control group and observation group according to the random number table method,with 44 cases in each group.The control group was given conventional care and the observation group was given evidence-based care.The psychological status,treatment compliance,quality of life and parental care satisfaction of patients in both groups were compared for 3 months of continuous intervention.Results:There was no statistically significant difference between the psychological status of the two groups before the intervention(P>0.05).The scores of anxiety(SAS)self-rating scale(40.32±5.75)points and depression(SDS)self-rating scale(42.51±6.57)points in the observation group were lower than(45.54±6.07)points and(47.70±5.19)points of in the control group after the intervention,with statistically significant differences(t=4.203,4.112,P<0.05).The treatment compliance in the observation group was 95.45%,which was higher than 79.55%in the control group,and the difference was statistically significant(χ^(2)=5.061,P<0.05).The difference in quality of life between the two groups before the intervention was not statistically significant(P>0.05).The physical(88.94±4.63)points,social(91.12±4.29)points,emotional(85.49±5.10)points and role(83.32±5.11)points on the Universal Core Rating Scale of Quality of Life(PedsQL4.0)for children in the observation group after the intervention were higher than(82.43±5.54)points(83.86±5.47)points,(76.31±5.62)points,and(76.44±5.20)points in the control group,with statistically significant differences(t=5.981,6.928,8.025,6.260,P<0.05).Parental satisfaction with care in the observation group was 95.45%,higher than 81.82%in the control group,with a statistically significant difference(χ^(2)=54.062,P<0.05).Conclusion:Evidence-based care can improve the psychological status of school-aged children with ITP,enhance treatment compliance and family care satisfaction,and improve the quality of life of children.
