神经内镜辅助脑室内出血的微创手术临床分析

Clinical Analysis of Neuroendoscopy-assisted Minimally Invasive Surgery for Intraventricular Hemorrhage

目的:总结、探讨神经内镜辅助治疗脑室内出血的疗效及优势。方法:回顾性分析2016年1月—2021年1月南通大学附属海门医院应用神经内镜微创清除脑室内血肿+外引流的方法、治疗21例脑室内出血患者的临床资料。结果:术后CT显示,侧脑室内血肿大部分清除16例,部分清除5例。术后随访3~6个月,GOS分级:Ⅳ~Ⅴ级18例,Ⅲ级2例,Ⅱ级1例。结论:应用神经内镜清除脑室内血肿+外引流治疗脑室内出血,具有直视下操作、血肿清除满意、手术损伤小、术后并发症少,脑脊液引流通畅的优点,是一种安全有效的外科治疗方法。

Objective:To summarize and discuss the efficacy and advantages of neuroendoscopic-assisted treatment of intraventricular hemorrhage.Methods:The clinical data of 21 patients with intraventricular hemorrhage from January 2016 to January2021 treated with neuroendoscopic minimally invasive removal of intraventricular hematoma + external drainage were analyzed.Results:Postoperative CT showed that the lateral intraventricular hematoma was mostly cleared in 16 cases and partially cleared in 5cases.At postoperative follow-up from 3 to 6 months,18 cases were graded as GOS grade Ⅳ-Ⅴ,2 cases as grade Ⅲ,and 1 case as grade Ⅱ.Conclusion:The application of neuroendoscopic removal of intracerebroventricular hematoma + external drainage for the treatment of intracerebroventricular hemorrhage has the advantages of operating under direct vision,satisfactory hematoma removal,minimal surgical injury,few postoperative complications,and unobstructed cerebrospinal fluid drainage,which is a safe and effective surgical treatment.
