高流量湿化治疗仪结合丹红注射液在慢性阻塞性肺疾病急性加重期合并Ⅱ类呼衰患者中的应用价值研究

摘要 目的:探究高流量湿化治疗仪结合丹红注射液在慢性阻塞性肺疾病急性加重期(AECOPD)合并Ⅱ类呼吸衰竭患者临床治疗中的应用价值。方法:选取2019年7月—2021年6月河南省方城县中医院收治的74例AECOPD合并Ⅱ类呼衰患者作为研究对象,根据随机数表法分为研究组与对照组,每组各37例。对照组采用高流量湿化治疗仪,研究组采用高流量湿化治疗仪结合丹红注射液。对比两组患者慢性阻塞性肺疾病(COPD)病情评价、动脉血气指标、炎性因子水平、内皮缩血管肽-1 (ET-1)与N端脑钠肽前体(NR-proBNP)水平、治疗效果及不良反应发生情况。结果:治疗后,研究组CAT评分低于对照组,差异有统计学意义(t=5.379,P<0.05)。治疗后,研究组血氧饱和度(SpO_(2))、动脉血氧分压(PaO_(2))水平均高于对照组,研究组动脉血二氧化碳分压(PaCO_(2))水平低于对照组,差异有统计学意义(t=17.454、12.655、8.965,P<0.05)。治疗后,研究组IL-6、TNF-α、NLR、ET-1、NR-proBNP水平均低于对照组,差异有统计学意义(t=10.394、18.548、6.890、26.116、15.100,P<0.05)。研究组治疗总有效率高于对照组,差异有统计学意义(χ^(2)=9.946,P<0.05)。两组患者不良反应总发生率比较,差异无统计学意义(χ^(2)=1.057,P>0.05)。结论:高流量湿化治疗仪结合丹红注射液应用于AECOPD合并Ⅱ类呼吸衰竭患者中,能够显著改善患者的COPD症状,有利于促进其血气指标、炎性因子、ET-1及NR-proBNP水平的恢复,为患者带来更加理想的治疗效果。 Objective:To investigate the value of high-flow humidification therapy instrument combined with Danhong injection in the clinical treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)combined with class II respiratory failure.Methods:74 patients with AECOPD combined with class II respiratory failure admitted to the hospital from July 2019 to June 2021 were selected as study subjects and were divided into study group and control group according to the random number table method,with 37 cases in each group.The control group used high-flow wetting therapy device,and the study group used high-flow wetting therapy device combined with Danhong injection.The evaluation of COPD,arterial blood gas index,inflammatory factor level,ET-1 and NR-proBNP level,treatment effect and occurrence of adverse effects were compared between the two groups.Results:After treatment,the CAT score of the study group was lower than that of the control group,and the difference was statistically significant(t=5.379,P<0.05).After treatment,SpO_(2) and PaO_(2) levels in the study group were higher than those in the control group,and PaCO_(2) levels in the study group were lower than those in the control group,with statistically significant differences(t=17.454,12.655,8.965,P<0.05).After treatment,the levels of IL-6,TNF-α,NLR,ET-1,and NR-proBNP were lower in the study group than in the control group,with statistically significant differences(t=10.394,18.548,6.890,26.116,15.100,P<0.05).The total effective rate of treatment in the study group was higher than that in the control group,and the difference was statistically significant(χ^(2)=9.946,P<0.05).There was no statistically significant difference in the overall incidence of adverse reactions between the two groups(χ^(2)=1.057,P>0.05).Conclusion:The application of high-flow humidification therapy device combined with Danhong injection in patients with AECOPD combined with class II respiratory failure can significantly improve patients􀆳symptoms of COPD and help promote the recovery of their blood gas index,inflammatory factor,ET-1 and NR-proBNP levels,which brings more ideal treatment results for patients.
