ICU转出早期重型颅脑创伤患者家属迁移应激现况及影响因素分析

Relocation stress and its influencing factors in family members of patients with Severe Traumatic Brain Injury transferred out of ICU

目的:分析ICU转出早期重型颅脑创伤患者家属迁移应激现况及其影响因素,为制定有针对性的干预措施提供参考依据。方法:便利选取2021年11月至2022年8月从上海市某3家综合医院ICU转出的重型颅脑创伤患者家属共155名作为研究对象,采用一般情况调查表、ICU转出患者家属迁移应激量表、社会支持评定量表、家属照顾能力量表及简易应对方式量表进行调查。结果:ICU转出早期重型颅脑创伤患者家属迁移应激总分为(60.66±1.64)分;家属的性别、文化程度、ICU探视次数、社会支持、积极应对是ICU转出早期重型颅脑创伤患者家属迁移应激的主要影响因素(P<0.05),共同解释其71.6%的变异水平。结论:ICU转出早期重型颅脑创伤患者家属的迁移应激处于中等偏上水平,医护人员应注重评估ICU转出早期家属的心理状况和需求,并根据影响因素制定个性化的干预措施,预防家属迁移应激的发生,或减轻其迁移应激水平。

Objective:To explore the current status and influencing factors of relocation stress in family members of patients with Severe Traumatic Brain Injury in the early stage transferred out of ICU.Methods:From November 2021 to August 2022,convenience sampling was used to select 155 family members of patients with Severe Traumatic Brain Injury transferred out of ICU in three general hospitals in Shanghai.The general information questionnaire of patients and their family members,Family Relocation Stress Syndrome Scale,Social Support Rating Scale,Family Caregiver Task Inventory,and Simplified Coping Style Questionnaire were used for the survey.Results:The total score of relocation stress was 60.66±1.64.Family members'gender,family members'educational level,family members'visits to ICU,social support and positive coping were the main influencing factors of relocation stress in ICU patients with Severe Traumatic Brain Injury(P<0.05),which together explained 71.6%of the total variation.Conclusion:The relocation stress of family members of patients with Severe Traumatic Brain Injury is at the medium upper level.Medical professionals should assess the psychological status and needs of family members in the early stage of ICU transfer,and formulate personalized interventions according to the influencing factors,so as to prevent the occurrence of relocation stress of family members.
