吴茱萸汤加味联合前庭康复训练预防性治疗前庭性偏头痛临床观察

Observation of Fructus Evodiae Decoction Combined with Vestibular Rehabilitation Training for Preventive Treatment of Vestibular Migraine

目的 通过探索吴茱萸汤加味联合前庭康复训练预防性治疗,观察前庭性偏头痛临床疗效。方法 选取符合纳入标准前庭性偏头痛患者100例,随机分为治疗组和对照组,各50例,同时予生活方式指导。治疗组口服吴茱萸汤加味4周,前庭康复训练3月。对照组口服氟桂利嗪10 mg,每晚1次,疗程3月。治疗前、治疗后第1周、第4周予眩晕残障程度评定量表(DHI)、中医眩晕评定量表评估;治疗后4周、3月评估眩晕头痛严重程度评分、持续时间评分;3月后评估发作频率及疗效。结果 2组治疗均有效,但治疗后中医眩晕评定量表评分、DHI评分、眩晕头痛发作频率、发作程度,治疗组比对照组改善更明显(P<0.05)。结论 吴茱萸汤加味联合前庭康复训练可预防性改善前庭性偏头痛患者眩晕、头痛发作频率和程度。

Objective:To observe the clinical effect of vestibular migraine treatment by exploring the preventive treatment of Fructus Evodiae Decoction combined with vestibular rehabilitation training.Methods:100 vestibular migraine patients who met the inclusion criteria were randomly divided into treatment group and control group,50 cases each group.The treatment group received oral administration of Fructus Evodiae Decoction for 4 weeks and vestibular rehabilitation training for 3 months.The control group was given fluunarizine 10 mg orally,once a night for 3 months.Vertigo Disability Rating Scale(DHI)and TCM Vertigo Rating Scale were assessed before treatment,and at the 1st and 4th week after treatment.Severity score and duration score of vertigo and headache were evaluated 4 weeks and 3 months after treatment.The attack frequency and curative effect were evaluated 3 months later.Results:The treatment was effective in both groups,but after treatment,the score of TCM vertigo rating scale,DHI score,frequency and severity of vertigo and headache were improved more significantly in the treatment group than in the control group(P<0.05).Conclusion:Fructus Evodiae Decoction combined with vestibular rehabilitation training can prophylactically improve the frequency and degree of vertigo and headache in patients with vestibular migraine.
