中性粒细胞/淋巴细胞比值与不同病变部位脑白质疏松的相关性研究

The correlation between neutrophil/lymphocyte ratio and white matter osteoporosis in different lesion sites

目的探讨中性粒细胞/淋巴细胞比值与不同病变部位脑白质疏松(皮质下及脑室旁)的相关性。方法本研究以2021年7月至2022年4月行头部磁共振成像检查并确诊为脑白质疏松症的老年患者60例为研究对象,根据病变部位不同将其分为皮质下病变组和脑室旁病变组(n=30),再根据头部磁共振成像检查的严重程度分为轻、中、重组,统计皮质下病变组和脑室旁病变组的一般资料、血细胞计数、同型半胱氨酸以及中性粒细胞/淋巴细胞比值,以Logistic回归性分析中性粒细胞/淋巴细胞比值与皮质下病变、脑室旁病变的相关性,受试者工作特征曲线(ROC)评估中性粒细胞/淋巴细胞比值的预测价值。结果脑室旁病变组,脑白质疏松轻度、中度、重度病变患者的年龄、高血压病史存在明显差异(P<0.05)。皮质下病变组,脑白质疏松轻度、中度、重度病变患者的年龄、高血压病史以及中性粒细胞/淋巴细胞比值存在明显差异(P<0.05)。Logistic回归性分析显示,年龄、高血压病史以及中性粒细胞/淋巴细胞比值为皮质下病变的危险因素(P<0.05)。ROC曲线分析显示,中性粒细胞/淋巴细胞比值预测的脑白质疏松的AUC为0.893,特异度、灵敏度分别为75.45%、58.59%。结论脑白质疏松患者中,皮质下病变、脑室旁病变的不同病变程度患者的年龄、高血压病史等一般资料有所差异,中性粒细胞/淋巴细胞比值是皮质下病变的独立危险因素。

Objective To investigate the correlation between neutrophil/lymphocyte ratio and white matter osteoporosis(subcortical and paraventricular)in different lesion sites.Methods A total of 60 cases of leukomatosis patients who received head magnetic resonance imaging examination in our hospital from July 2021 to April 2022 was included as research objects.According to different lesion sites,they were divided into subcortical lesion group and paraventricular lesion group(n=30),and then grouped according to the severity of head magnetic resonance imaging examination.The general data,blood count,homocysteine and neutrophil/lymphocyte ratio of the subcortical lesions and the para-ventricular lesions were analyzed by Logistic regression,and the correlation between the ratio of neutrophil/lymphocyte and the subcortical lesions and para-ventricular lesions was analyzed.The predictive value of neutrophil/lymphocyte ratio was evaluated by receiver operating characteristic curve(ROC).Results There were significant differences in age and history of hypertension among patients with mild,moderate and severe leukoosteoporosis in paraventricular disease group(P<0.05).In the subcortical disease group,there were significant differences in age,hypertension history and neutrophil/lymphocyte ratio among patients with mild,moderate and severe leukoosteoporosis(P<0.05).Logistic regression analysis showed that age,hypertension history and neutrophil/lymphocyte ratio were risk factors for subcortical lesions(P<0.05).ROC curve analysis showed that the AUC predicted by the ratio of neutrophils/lymphocytes was 0.893,and the specificity and sensitivity were 75.45%and 58.59%,respectively.Conclusion The age,age,history of hypertension and other general data of patients with subcortical and paraventricular lesions varies in the patients with leukoosteoporosis.Neutrophil/lymphocyte ratio is an independent risk factor for subcortical lesions.
