基于心肺运动试验的有氧训练对脑卒中患者心肺功能及运动能力的疗效观察 被引量:1

摘要 目的观察基于心肺运动试验的有氧训练对脑卒中患者心肺功能及运动能力的疗效。方法本研究选取2018年3月至2020年6月我院收治的脑卒中患者40例,随机分为试验组和对照组,每组20例。对照组在常规康复治疗基础上增加非基于心肺运动试验的有氧训练;试验组在常规康复治疗基础上增加基于心肺运动试验的有氧训练。干预前和干预后第6周、第12周,采用心肺运动试验、Fugl-Meyer评定量表下肢部分、简易体能状况量表(SPPB)、6分钟步行试验(6MWT)分别对两组患者的心肺功能、下肢随意活动能力、跌倒风险和步行能力进行评定。结果干预后第6周和第12周试验组的峰值摄氧量、无氧阈摄氧量、峰值运动功率、运动测试时间、SPPB评分和6MWT距离均持续改善,且均显著高于对照组(P<0.05);干预后第6周和第12周在两组内Fugl-Meyer评定量表下肢部分评分均较前改善,但两组间无显著差异(P>0.05)。结论基于心肺运动试验的有氧训练可以有效改善脑卒中患者心肺功能和运动能力。 Objective To observe the effect of aerobic training based on cardiopulmonary exercise test on cardiopulmonary function and exercise capacity of stroke patients Methods A total of 40 stroke patients admitted to our hospital from March 2018 to June 2020 were selected and randomly divided into an experimental group and a control group,20 cases in each group.The control group added non-cardiopulmonary exercise test-based aerobic training on the conventional rehabilitation therapy,and the experimental group added cardiopulmonary exercise test-based aerobic training on the conventional rehabilitation therapy.Before the intervention,at the 6th week and the 12th week,cardiopulmonary exercise test,Fugl-Meyer Rating Scale lower extremity part,Simple physical condition scale(SPPB)and 6-minute walk test(6MWT)were used to evaluate the cardiopulmonary function,voluntary activity of lower limbs,risk of falls and walking ability of patients in the two groups.Results The peak oxygen uptake,anaerobic threshold oxygen uptake,peak exercise power,exercise test time,SPPB score and 6MWT distance of the experimental group continued to improve at the 6th week and 12th week,and were significantly higher than those of the control group(P<0.05);at the 6th and 12th week,the scores of the lower extremity of the Fugl-Meyer Rating Scale in the two groups were all improved compared with before,but there was no significant difference between the two groups(P>0.05).Conclusion Aerobic training based on cardiopulmonary exercise test can effectively improve cardiopulmonary function and exercise capacity in stroke patients.
