瑞舒伐他汀钙治疗对颈动脉粥样硬化患者斑块及血脂水平的影响

Effects of rosuvastatin calcium treatment on plaque and blood lipid levels in patients with carotid atherosclerosis

目的观察瑞舒伐他汀钙治疗对颈动脉粥样硬化患者斑块及血脂水平的影响。方法179例颈动脉粥样硬化患者中,133例使用瑞舒伐他汀钙治疗(观察组),46例拒绝使用他汀类药物治疗(对照组),两组基础治疗均为阿司匹林肠溶片。比较两组治疗前和治疗1、2年的血脂水平及颈动脉斑块积分变化。结果治疗1年时,观察组TC、TG和LDL-C较治疗前降低,载脂蛋白A1(ApoA1)较治疗前升高(P<0.05);治疗2年时上述指标改变较治疗1年更为显著(P<0.05)。治疗1年时,对照组TC、TG、ApoA1、ApoB、脂蛋白a(LPa)、HDL-C和LDL-C与治疗前比较差异无统计学意义(P>0.05);治疗2年时,TC、ApoB和LDL-C较治疗前和治疗1年时均升高(P<0.05)。治疗1年时,观察组颈动脉斑块积分与治疗前相仿(P>0.05);治疗2年时颈动脉斑块积分较治疗前降低,但差异无统计学意义(P>0.05)。对照组治疗1年时颈动脉斑块积分较治疗前升高(P<0.05),治疗2年时动脉斑块积分较治疗前和治疗1年时升高(P<0.05)。结论瑞舒伐他汀钙具有稳定粥样硬化斑块和改善血脂水平的作用。

Objective To observe the effects of rosuvastatin calcium treatment on plaque and blood lipid levels in the patients with carotid atherosclerosis.Methods Of 179 patients with carotid atherosclerosis,133 cases were treated with rosuvastatin calcium(group A)and 46 cases were not(group B).The basic treatment of two groups was aspirin enteric coated tablets.The changes in blood lipid levels and carotid plaque scores were compared between the two groups at the time points of before treatment,1 year and 2 years of treatment.Results At 1 year of treatment,the levels of TC,TG,and LDL-C in group A were decreased,while the ApoA1 level was increased compared to those before treatment(P<0.05),which at 2 years of treatment in group A were more obvious than those at 1 year of treatment(P<0.05).There were no statistically differences of TC,TG,ApoA1,ApoB,LPa,HDL-C and LDL-C levels between 1 year of treatment and before treatment in group B(P<0.05).At 2 years of treatment,the levels of TC,ApoB and LDL-C were increased in group B compared to those before treatment and 1 year of treatment(P<0.05).At 1 year of treatment,there was no significant change in the carotid plaque score of group A compared to that before treatment(P<0.05).At 2 years of treatment,the carotid plaque score of group A was decreased compared to that before treatment,but the difference was not statistically significant(P<0.05).The carotid plaque score in group B was increased at 1 year of treatment compared to that before treatment(P<0.05),which was significantly increased in group B at 2 years of treatment compared to that before treatment and 1 year of treatment(P<0.05).Conclusion Rosuvastatin calcium can stabilize atherosclerotic plaque and improve blood lipid levels.
