临床医生对DRG支付改革的知信行研究

Investigation on clinicians’knowledge,belief and practice of DRG payment reform

目的调查临床医生对DRG支付改革的知信行现状,探索DRG支付改革对医生可能产生的影响。方法2021年7月20—31日采用在线调查,通过一般资料调查表、知信行调查量表对来自某市的纳入DRG支付试点改革的临床医生进行信息收集。结果临床医生对于DRG支付改革的知识、信念、行为得分分别为(72.12±12.64)分、(43.94±7.13)分和(36.84±7.20)分。多元线性回归分析结果显示,知识影响因素包括学历、科室类别、培训频次;信念影响因素包括知识、是否参与过培训;行为影响因素包括知识、信念、培训频次。结论多角度开展宣传培训,提高临床医生的知信行水平;结合医院制度改革,探索绩效考核理想发展;加强成本控制管理,优化临床路径。

Objective To investigate the current situation of medical staff’s knowledge and practice on the reform of DRG and explore the possible influence of doctors on the reform of DRG.Methods From July 20to July 31,2021,general data questionnaire and knowledge,trust and practice questionnaire were used to collect the information of medical personnel from a city who were included in the DRG payment pilot reform.Results The scores of knowledges,belief,and behavior of medical staff for DRG reform were(72.12±12.64)points,(43.94±7.13)points and(36.84±7.20)points respectively.Conclusion Carry out publicity training from multiple angles to improve the knowledge,trust and practice level of medical staff;Combined with hospital system reform,explore the ideal development of performance appraisal;Strengthen cost control management and optimize clinical pathways.
