注射用唑来膦酸药物利用评价标准的建立与应用

Establishment and application of evaluation criteria for drug utilization of zoledronic acid for injection

目的:建立注射用唑来膦酸的药物利用评价标准,指导其临床合理应用。方法:参考药品说明书及相关指南,通过分层加权TOPSIS法对安徽省太和县人民医院2019年9月至2022年9月161例使用注射用唑来膦酸的出院病历进行分析和评估。结果:所评价的161份病历中,完全符合标准的共22例(占13.66%),其主要不合理表现为适应证不适宜(42例,26.09%)、辅助检查及实验室检查未完善(33例,20.50%)、未进行用药监护(29例,18.01%)以及不良反应未进行监测和上报(21例,13.04%)等;所纳病历中,合理病历(C_(i)≥0.8)占比为16.77%、基本合理病历(0.6≤C_(i)<0.8)占比为65.83%、不合理病历(C_(i)<0.6)占比为17.39%。结论:基于加权TOPSIS法建立的注射用唑来膦酸合理性评价方法方便、可靠、直观,该院仍需进一步规范注射用唑来膦酸的临床应用。

OBJECTIVE To establish the evaluation criteria for drug utilization of zoledronic acid for injection and provide reference for clinical rational application.METHODS Based on the drug inserts and related guidelines,161 cases of patients treated with zoledronic acid for injection in Taihe Courcty people’s Hospital from September 2019 to September 2022 were analyzed by using the stratified weighted TOPSIS method.RESULTS Among the 161 medical records evaluated,22 cases(13.66%)fully met the criteria.The main unreasonable manifestations were inappropriate indications(42 cases,26.09%),incomplete auxiliary examinations and laboratory examinations(33 cases,20.50%),no medication monitoring(29 cases,18.01%)and adverse reactions were not monitored and reported(21 cases,13.04%).The reasonable medical records(C_(i)≥0.8)accounted for 16.77%,basically reasonable medical records(0.6≤C_(i)<0.8)accounted for 65.83%,and unreasonable medical records(C_(i)<0.6)accounted for 17.39%.CONCLUSION The rationality evaluation method for zoledronic acid for injection based on the weighted TOPSIS method is convenient,reliable and intuitive.The application of zoledronic acid for injection still needs to be further standardized in hospital.
