基于间断时间序列分析处方前置审核的实施对门诊处方合格率的影响

Analysis of the implementation of prescription pre-review on outpatient prescription qualification rate based on interrupted time series

目的:系统评价处方前置审核的实施对门诊处方合格率的影响,为临床合理、有效和安全用药提供循证药学参考。方法:利用合理用药分析软件,收集青岛市城阳区人民医院2021年2月1日至2022年5月31日的门诊处方合格率,并利用间断时间序列分析法对其进行分析。结果:处方前置审核实施后第一个月(2021年10月),门诊处方合格率显著上升6.352%(P<0.01,95%CI:4.432~8.272);与实施前趋势相比,门诊处方合格率增加了0.348%(P=0.024,95%CI:0.055~0.641);参数线性组合结果显示,处方前置审核实施后,每月门诊处方合格率以0.472%(P=0.008,95%CI:0.147~0.798)趋势上升。结论:实施处方前置审核,提高了门诊处方的合格率,促进了临床合理用药。

OBJECTIVE To systematically evaluate the impact of the implementation of prescription pre-audit on the qualified rate of outpatient prescriptions,and provide evidence-based pharmaceutical reference for clinical rational,effective and safe drug use.METHODS Using the rational drug use analysis system,the qualified rates of outpatient prescriptions were collected from February 1,2021 to May 31,2022 in Chengyang District People’s Hospital of Qingdao,and the interrupted time series was used for the analysis.RESULTS In the first month after the implementation of prescription pre-audit(October 2021),the qualified rate of outpatient prescription increased significantly by 6.352%(P<0.01,95%CI:4.432-8.272),and compared to the pre-implementation trend,the qualified rate of outpatient prescription increased by 0.348%(P=0.024,95%CI:0.055-0.641).The results of the linear combination of parameters showed that,after the implementation of prescription pre-review,the monthly qualification rate of outpatient prescriptions increased with a trend of 0.472%(P=0.008,95%CI:0.147-0.798).CONCLUSION The implementation of prescription pre-review can effectively improve the qualified rate of outpatient prescriptions,and promote the clinical rational drug use.
