基于问题学习结合标准化病人教学方法在慢性骨髓炎诊治教学中的应用

摘要 目的探讨基于问题学习(problem-based learning,PBL)结合标准化病人(standardized patient,SP)教学方法在慢性骨髓炎诊治教学中的应用效果。方法选择2020年9月至2022年8月在浙江大学医学院附属邵逸夫医院骨科进行住院医师规范化培训的56名学员为研究对象。采用随机试验对照研究方法,将56名住培学员分为试验组和对照组,每组28名住培学员。试验组住培学员采用PBL结合SP教学方法,对照组住培学员采用传统教学方法。4周的培训结束后,对两组住培学员进行慢性骨髓炎理论知识和实践操作考核,以及有关教学效果的问卷调查。结果试验组住培学员慢性骨髓炎综合诊治理论知识成绩、实践操作成绩和总成绩,以及对教学方法的认可度、自我学习积极性、合作与沟通能力等方面的评分均优于对照组住培学员,其差异均具有统计学意义(均P<0.05)。结论PBL结合SP教学方法能够有效提高试验组住培学员有关慢性骨髓炎综合诊治的各项考核成绩,提高其对教学效果的评分,值得在今后的住院医师规范化培训中进一步推广。 Objective To explore the application effect of problem-based learning(PBL)combined with standardized patient(SP)teaching method in the teaching of diagnosis and treatment of chronic osteomyelitis.Methods From September 2020 to August 2022,56 trainees who received standardized resident training in Department of Orthopedics,Sir Run Run Shaw Hospital Affiliated to Zhejiang University School of Medicine were selected as the research objects.According to the random principle,56 trainees were divided into experimental group and control group,with 28 trainees in each group.The students in the experimental group adopted PBL combined with SP teaching method,and the students in the control group adopted the traditional teaching method.After 4 week training,the two groups of trainees were assessed on the theory and operation of chronic osteomyelitis,as well as a questionnaire survey on the teaching effect.Results The students in the experimental group scored better than those in the control group in terms of theoretical knowledge,practical operation and total scores of comprehensive diagnosis and treatment of chronic osteomyelitis,as well as the recognition of teaching methods,self-learning enthusiasm,cooperation and communication skills,and the differences were statistically significant(all P<0.05).Conclusions PBL combined with SP teaching method can effectively improve the test scores of the students in the experimental group on the comprehensive diagnosis and treatment of chronic osteomyelitis,and improve their evaluation of the application effect of the teaching method,which is worthy of further promotion in clinical teaching in the future.
