直立脊柱减压技术治疗腰椎间盘突出症临床疗效观察

Clinical Observation on the Treatment of Lumbar Disc Herniation with Upright Spinal Decompression Technique

目的:观察直立脊柱减压技术对腰椎间盘突出症(LDH)患者腰部疼痛、腰椎功能障碍和脊神经功能的治疗效果。方法:选取2020年6月至2021年10月山东中医药大学第二附属医院康复医院门诊与病房收治的LDH患者100例,运用随机数字表法分为对照组和试验组,每组50例。对照组采用常规仰卧位腰椎牵引结合推拿治疗,30 min/次,1次/d,5 d/周,共治疗4周。试验组采用直立脊柱减压技术治疗,30 min/次,1次/d,5 d/周,共治疗4周。分别在治疗前、治疗4周后采用疼痛视觉模拟评分表(VAS)、日本骨科协会评估治疗分数(JOA)、腰椎Oswstry功能障碍指数(ODI)评定患者腰部疼痛程度、腰椎功能、脊髓神经功能。结果:治疗前,两组患者在VAS评分、JOA评分、ODI评分方面的差异性均无统计学意义(P>0.05)。治疗4周后,2组患者VAS评分、JOA评分、ODI评分均有改善(P<0.05),两组VAS评分、ODI评分均下降,差异均有统计学意义(P<0.05);试验组对腰部疼痛、功能障碍方面的治疗效果均优于对照组(P<0.05)。结论:直立脊柱减压技术相比仰卧位牵引配合推拿治疗在缓解LDH患者腰部疼痛、改善腰椎功能方面有更好的疗效。

Objective:To observe the therapeutic effect of upright spinal decompression on lumbar pain,lumbar dysfunction and spinal nerve function in patients with lumbar disc herniation(LDH).Method:From June 2020 to October 2021,100 patients with LDH treated in the outpatient and ward of the rehabilitation hospital of the Second Affiliated Hospital of Shandong University of traditional Chinese medicine were randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with conventional supine lumbar traction combined with massage,30 minutes per time,once a day,5 days per week,for a total of 4 weeks.The observation group was treated with upright spinal decompression technique,30min/time,once a day,5D/week,for a total of 4 weeks.Before and 4 weeks after treatment,the degree of lumbar pain,lumbar function and spinal nerve function were evaluated by Visual Analogue Scale(VAS),Japanese Orthopaedic Association evaluation treatment score(JOA)andOswstry Disability Index(ODI).Result:Before treatment,there was no significant difference in VAS score,JOA score and ODI score between the two groups(P>0.05).After 4 weeks of treatment,the VAS score,JOA score and ODI score of the two groups were improved(P<0.05),and the VAS score and ODI score of the two groups were decreased(P<0.05);The therapeutic effect of the experimental group on lumbar pain,dysfunction and Spinal nerve function were better than that of the control group(P<0.05).Conclusion:Compared with supine traction combined with massage,upright spinal decompression technology has better curative effect in alleviating lumbar pain,improving lumbar function and Spinal nerve function in patients with LDH.
