收肌管阻滞联合消肿止痛方外敷在膝关节置换术后镇痛中的应用

Application of Adductor Canal Block Combined with External Application of Xiaozhong Zhitong Formula in Postoperative Pain Relief after Knee Joint Replacement Surgery

目的:探究膝关节置换术患者采用收肌管阻滞联合消肿止痛方外敷的临床疗效。方法:选取2021年1月-2022年9月赣州市人民医院收治的60例膝关节置换术患者作为研究对象,采用随机数字表法将其分为对照组与观察组,各30例,均行膝关节置换术,对照组采用收肌管阻滞,观察组同时联合消肿止痛方外敷,比较两组视觉模拟评分量表(VAS)评分、不良反应发生率、美国膝关节协会(KSS)评分。结果:两组术后静息状态下8、16 h的VAS评分对比,差异均无统计学意义(P>0.05),观察组术后静息状态下24、32、40、48 h的VAS评分均低于对照组,差异均有统计学意义(P<0.05);观察组活动状态下各时间段VAS评分均低于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率对比,差异无统计学意义(P>0.05);两组治疗前KSS评分对比,差异无统计学意义(P>0.05);观察组治疗后KSS评分高于对照组,差异有统计学意义(P<0.05)。结论:膝关节置换术后患者给予收肌管阻滞联合消肿止痛方外敷疗法,可显著减轻机体疼痛,提高膝关节功能。

Objective:To explore the clinical efficacy of adductor canal block combined with external application of Xiaozhong Zhitong Formula in patients undergoing knee joint replacement.Method:A total of 60 patients with knee joint replacement admitted to People's Hospital of Ganzhou City from January 2021 to September 2022 were selected as research subjects.They were randomly divided into the control group and the observation group by random number table method.Each group had 30 patients undergoing knee joint replacement.The control group received adductor canal block,and the observation group was combined with external application of Xiaozhong Zhitong Formula.The scores of the visual analogue scale(VAS)and the incidence of adverse reactions,American knee association score(KSS)between the two groups were compared.Result:There were no significant differences in VAS scores between the two groups at resting state 8 and 16 hours after operation(P>0.05).The VAS scores at resting state 24,32,40 and 48 hours after operation in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The VAS scores of the observation group were lower than those of the control group at each time point in the active state,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).There was no statistically significant difference in KSS score between the two groups before treatment(P>0.05).The KSS score of the observation group was higher than that of the control group after treatment,and the difference was statistically significant(P<0.05).Conclusion:After knee joint replacement,the treatment of adductor canal block combined with external application of Xiaozhong Zhitong Formula can significantly reduce body pain and improve knee joint function.
