摘要
目的探讨彩色多普勒超声在下肢深静脉血栓(LEDVT)的诊断及评估溶栓效果中应用价值。方法选择2019年1月至2021年12月于我院行彩色多普勒超声、下肢静脉造影检查的疑似LEDVT患者80例,对静脉造影确诊者给予溶栓治疗,完成溶栓治疗后予以彩色多普勒超声复查。以静脉造影结果为金标准,分析彩色多普勒超声的诊断效能,对比溶栓治疗前LEDVT与非LEDVT患者的血流参数,并对比溶栓治疗后有效组、无效组的血流参数。结果以静脉造影结果为金标准,彩色多普勒超声诊断LEDVT的灵敏度、特异度、准确度分别为98.51%、84.62%、96.25%。LEDVT患者的EDV、PSV均高于非LEDVT患者,RI低于非LEDVT患者(P<0.05)。有效组的EDV、PSV均低于无效组,RI高于无效组(P<0.05)。结论彩色多普勒超声能够为临床判定LEDVT形成及预测溶栓效果提供可靠依据。
Objective To explore the application value of color Doppler ultrasound in the diagnosis of lower extremity deep vein thrombosis(LEDVT)and the evaluation of thrombolytic effect.Methods 80 suspected LEDVT patients with color Doppler ultrasound and lower limb phlebography from January 2019 to December 2021 in our hospital were selected.Those who were confirmed by phlebography received thrombolysis.After thrombolysis,they were rechecked by color Doppler ultrasound.Taking the venography results as the gold standard,the diagnostic efficacy of color Doppler ultrasound was analyzed,the blood flow parameters of LEDVT and non LEDVT patients before thrombolysis were compared,and the blood flow parameters of the effective group and the ineffective group after thrombolysis were compared.Results Taking the results of venography as the gold standard,the sensitivity,specificity and accuracy of color Doppler ultrasound in the diagnosis of LEDVT were 98.51%,84.62%and 96.25%,respectively.The EDV and PSV of LEDVT patients were higher than those of non LEDVT patients,and the RI was lower than that of non LEDVT patients(P<0.05).The EDV and PSV of the effective group were lower than those of the ineffective group,and the RI was higher than that of the ineffective group(P<0.05).Conclusions Color Doppler ultrasound can provide reliable basis for clinical judgement of LEDVT formation and prediction of thrombolytic effect.
