摘要
目的探讨2型糖尿病(type 2 diabetes mellitus,T2DM)患者肠道菌群对骨质疏松发生的影响。方法选取2020年3月—2021年12月杭州师范大学附属医院收治的T2DM患者68例,根据随访结果分为两组,2型糖尿病伴骨质疏松作为观察组,2型糖尿病不伴骨质疏松作为对照组,各34例。检测两组患者肠道菌群,测定患者第2~4腰椎(lumbar spine 2~4,L2~4)及左股骨颈的骨密度(bone mineral density,BMD),采用Pearson相关性分析肠道菌群与骨密度相关性。结果2型糖尿病骨质疏松组患者肠道菌群的β多样性与对照组比较,差异有统计学意义(P<0.05);Kouleothrix菌较对照组升高(P<0.05);相关性分析显示Kouleothrix菌与L2-4的骨密度(BMD)呈正相关(r=0.65,P<0.001)。结论2型糖尿病患者肠道菌群发生改变可能对糖尿病性骨质疏松产生影响,其为临床治疗提供一定的参考。
Objective To investigate the effect of intestinal flora on the occurrence of osteoporosis in patients with type 2 diabetes mellitus(T2DM).Methods Sixty-eight patients with T2DM admitted to the Affiliated Hospital of Hangzhou Normal University from March 2020 to December 2021 were selected.According to the follow-up results,they were divided into two groups,the type 2 diabetic osteoporosis group as the observation group and the type 2 dia-betic group without osteoporosis group as the control group,34 cases each.The intestinal flora of the two groups were measured,and the bone mineral density(BMD)of the lumbar spine 2-4(L2-4)and the left femoral neck were mea-sured,and the correlation between intestinal flora and BMD was analyzed by Pearson correlation.Resultsβdiversity of intestinal flora in type 2 diabetic osteoporosis group compared with control group,the difference was statistically sig-nificant(P<0.05).Kouleothrix bacteria were elevated compared with the control group(P<0.05).Correlation analysis showed a positive correlation between Kouleothrix bacteria and bone mineral density(BMD)in L2-4(r=0.65,P<0.001).Conclusion The alteration of intestinal flora in type 2 diabetic patients may have an impact on diabetic osteo-porosis,and it provides some reference for clinical treatment.
