眼底激光结合羟苯磺酸钙胶囊治疗DR的有效性、安全性及对视力指标的影响

Effectiveness and Safety of Fundus Laser Combined with Calcium Hy⁃droxybenzene Sulfonate Capsules in the Treatment of DR and the Effect on Visual Acuity Index

目的眼底激光结合羟苯磺酸钙胶囊治疗糖尿病性视网膜病变(diabetic retinopathy,DR)的有效性、安全性及对视力指标的影响。方法选取2020年1月—2021年12月无锡惠山区人民医院收治的DR患者82例,根据治疗方法分为观察组(40例)与对照组(42例)。对照组患者行视网膜激光光凝术,观察组在此基础上给予羟苯磺酸钙胶囊。比较两组治疗前后最佳矫正视力(BCVA)、黄斑厚度、黄斑区渗漏面积、眼底微循环[视网膜中央及睫状后动脉的舒张末期血流速度(EDV)、峰值血流速度(PSV)、阻力指数(RI)]、血管内皮功能指标[低氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)]及生活质量,并对比并发症发生情况。结果与对照组相比,观察组患者治疗后BCVA更高,黄斑厚度更低,黄斑区渗漏面积更小,视网膜中央及睫状后动脉的EDV、PSV均升高,RI、HIF-α及VEGF水平均更低,且生活质量评分更高,差异有统计学意义(P<0.05)。治疗期间两组并发症发生率比较,差异无统计学意义(P>0.05)。结论眼底激光结合羟苯磺酸钙胶囊可减轻DR患者临床症状,提高视力水平,改善患者血液循环,抑制血管增生,患者生活质量更高,且无严重并发症。

Objective The effectiveness and safety of fundus laser combined with calcium hydroxybenzene sulfonate capsules in the treatment of diabetic retinopathy(DR)and the effect on visual acuity indexes.Methods 82 patients with DR admitted to the Huishan District People's Hospital,Wuxi,from January 2020 to December 2021 were se-lected and divided into an observation group(40 cases)and a control group(42 cases)according to the treatment method.The control group underwent retinal laser photocoagulation,while the observation group received calcium do-besilate capsules on this basis.Compared the best corrected visual acuity(BCVA),macular thickness,macular leak-age area,fundus microcirculation[end diastolic blood flow velocity of the central retina and posterior ciliary artery(EDV),peak systolic blood flow velocity(PSV),resistance index(RI)],vascular endothelial function indicators[hy-poxia inducible factor-1α(HIF-1α),vascular endothelial growth factor(VEGF)],and quality of life between the two groups before and after treatment,and compared the incidence of complications.Results After treatment,compared with the control group,the observation group had higher BCVA,lower macular thickness,smaller leakage area in the macular area,higher EDV and PSV in the central retina and posterior ciliary artery,RI,HIF-1αand VEGF were lower and the quality of life score was higher,the difference was statistically significant(P<0.05).There was no statis-tically significant difference in the incidence of complications between the two groups during the treatment period(P>0.05).Conclusion Fundus laser combined with calcium hydroxybenzene sulfonate capsules can reduce clinical symp-toms,improve visual acuity level,improve blood circulation and inhibit vascular proliferation in patients with DR,and patients have higher quality of life without serious complications.
