超声引导下射频消融术联合乙醇消融对甲状腺良性囊实性结节患者甲状腺功能及应激反应的影响

Effects of ultrasound-guided radiofrequency ablation combined with ethanol ablation on thyroid function and stress response in patients with benign cystic solid nodule of thyroid gland

目的探讨超声引导下射频消融术联合乙醇消融治疗甲状腺良性囊实性结节的临床效果。方法择取2019年6月至2021年8月我院收治的85例甲状腺良性囊实性结节患者为研究对象,根据治疗方案将其分为对照组(42例,超声引导下射频消融术)和观察组(43例,超声引导下射频消融术联合乙醇消融)。比较两组的治疗效果。结果观察组的结节体积缩小率高于对照组,射频消融时间短于对照组,使用能量小于对照组(P<0.05)。术后,两组的甲状腺刺激抗体(TSAb)、甲状腺刺激阻断性抗体(TSBAb)、促甲状腺激素受体抗体(TRAb)水平均低于术前,且观察组低于对照组(P<0.05)。术后,两组的去甲肾上腺素(NE)、皮质醇(COR)、前列腺素E_(2)(PGE_(2))水平均高于治疗前,但观察组低于对照组(P<0.05)。结论超声引导下射频消融术联合乙醇消融治疗甲状腺良性囊实性结节不仅能有效缩小结节体积,还能缓解机体应激反应,促进甲状腺功能恢复。

Objective To investigate the clinical effect of ultrasound-guided radiofrequency ablation combined with ethanol ablation in the treatment of benign cystic solid nodule of thyroid gland.Methods Eighty-five patients with benign cystic solid nodule of thyroid gland admitted in our hospital from June 2019 to August 2021 were selected as the study objects and divided into control group(42 cases,ultrasound-guided radiofrequency ablation)and observation group(43 cases,ultrasound-guided radiofrequency ablation combined with ethanol ablation)according to the treatment plan.The therapeutic effects of the two groups were compared.Results The reduction rate of nodule volume in the observation group was higher than that in the control group,the radiofrequency ablation time was shorter than that in the control group,and the energy used was smaller than that in the control group(P<0.05).After operation,the levels of thyroid stimulating antibody(TSAb),thyrotropin stimulation-blocking antibody(TSBAb)and thyrotropin receptor antibody(TRAb)in both groups were lower than those before operation,and those in the observation group were lower than the control group(P<0.05).After operation,the levels of norepinephrine(NE),cortisol(COR)and prostaglandin E_(2)(PGE_(2))in both groups were higher than those before treatment,but those in the observation group were lower than the control group(P<0.05).Conclusion Ultrasound-guided radiofrequency ablation combined with ethanol ablation in the treatment of benign cystic solid nodule of thyroid gland can effectively reduce the nodule volume,relieve the stress response and promote the recovery of thyroid function.
