芪苈强心胶囊联合卡格列净治疗气虚血瘀证慢性心力衰竭合并2型糖尿病的临床研究

Effect of Qiliqiangxin Capsule Combined with Canagliflozin on Chronic Heart Failure with Qi Deficiency and Blood Stasis Syndrome Complicated with Type 2 Diabetes Mellitus

目的:研究芪苈强心胶囊联合卡格列净治疗气虚血瘀证型慢性心力衰竭合并2型糖尿病的疗效及安全性。方法:选择河南大学第一附属医院2022年1月至2022年6月诊治的56例气虚血瘀证慢性心力衰竭合并2型糖尿病患者,给予芪苈强心胶囊联合卡格列净治疗,比较治疗前后空腹血糖(FBG)、糖化血红蛋白(HbA1c)、左心室舒张末期内径(LVEDD)、左室射血分数(LVEF)、Tei指数、肌酸激酶同工酶MB(CK-MB)、超敏心肌肌钙蛋白I(hs-cTnI)、N末端B型利钠肽原(NT-proBNP)、可溶性肿瘤生长抑制因子2(sST2)及基质金属蛋白酶-2(MMP-2)的差异。结果:患者治疗后FBG、HbA1c、LVEDD及Tei指数均低于治疗前,LVEF高于治疗前,差异具有统计学意义(P<0.05);患者治疗后血清CK-MB、hs-cTnI、NT-proBNP、sST2、MMP-2水平均低于治疗前,差异具有统计学意义(P<0.05);患者治疗后身体领域、情绪领域、其他领域评分均低于治疗前,差异具有统计学意义(P<0.05)。结论:芪苈强心胶囊联合卡格列净治疗气虚血瘀证慢性心力衰竭合并2型糖尿病可改善患者FBG、HbA1c、心功能、心力衰竭标志物及生存质量,高效安全。

Objective To investigate the efficacy and safety of Qiliqiangxin capsule combined with Canagliflozin in the treatment of chronic heart failure with Qi deficiency and blood stasis syndrome combined with type 2 diabetes mellitus.Methods 56 patients with chronic heart failure with Qi deficiency and blood stasis syndrome combined with Type 2 diabetes,treated by the First Affiliated Hospital of Henan University from January 2022 to June 2022,were given Qiliqiangxin capsule combined with Canagliflozin.Fasting blood glucose(FBG),glycated hemoglobin(HbA1c),left ventricular end-diastolic inner diameter(LVEDD),left ventricular ejection fraction(LVEF),Tei index,creatine kinase isoenzyme MB(CK-MB),hypersensitive cardiac troponin I(hs-cTnI)and N-terminal pro-B type natriuretic peptide(NT-proBNP),soluble tumor growth inhibitory factor 2(sST2)and matrix metalloproteinase 2(MMP-2)were compared before and after treatment.Results After treatment,the levels of FBG,HbA1c,LVEDD and Tei indexes were lower than those before treatment,while LVEF was higher than that before treatment,the differences were statistically significant(P<0.05).After treatment,the levels of CK-MB,hs-cTnI,NT-proBNP,sST2,MMP-2 were lower than those before treatment,and the differences were statistically significant(P<0.05).After treatment,the scores of patients in the physical field,emotional field and other fields were lower than those before treatment,and the differences were statistically significant(P<0.05).Conclusion Qiliqiangxin capsule combined with Canagliflozin is effective and safe for treatment of chronic heart failure with Qi deficiency and blood stasis syndrome combined with type 2 diabetes,which improves FBG,HbA1c,cardiac function,heart failure markers and quality of life.
