单孔及三孔全胸腔镜入路手术治疗早期非小细胞肺癌患者的价值

Value of Single-Hole and Three-Hole Thoracoscopic Approach in the Treatment of Patients with Early Non-Small Cell Lung Cancer

目的:探究单孔及三孔全胸腔镜入路手术治疗早期非小细胞肺癌患者的价值。方法:选取河南科技大学第一附属医院2021年9月至2022年9月收治的早期非小细胞肺癌患者76例作为研究对象,采用随机数字表法分为对照组、观察组,各38例。对照组患者行三孔全胸腔镜入路手术,观察组行单孔全胸腔镜入路手术。比较两组患者的临床各项指标、免疫功能、肺功能、并发症发生情况。结果:观察组患者各项临床指标均低于对照组,差异具有统计学意义(P<0.05)。术后1 d,观察组患者外周血CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均高于对照组,CD8^(+)低于对照组,差异具有统计学意义(P<0.05)。术后2个月,观察组患者肺功能指标均高于对照组,差异具有统计学意义(P<0.05)。观察组患者并发症发生率低于对照组,差异具有统计学意义(P<0.05)。结论:相较于三孔全胸腔镜入路手术,单孔全胸腔镜入路手术的应用,对提高早期非小细胞肺癌患者治疗效果的优势更为明显,能够减轻患者受到的免疫损伤,并且还可改善其肺功能,降低并发症发生率。

Objective To explore the value of single and three-hole thoracoscopic approach in the treatment of patients with early non-small cell lung cancer.Methods A total of 76 patients with early non-small cell lung cancer admitted to the First Affiliated Hospital of Henan University of Science and Technology from September 2021 to September 2022 were selected as the study objects,and were divided into a control group and an observation group by random number table method,with 38 cases in each group.The control group underwent three-hole thoracoscopic approach,while the observation group underwent single-hole thoracoscopic approach.The clinical indexes,immune function,lung function and complications of the two groups were compared.Results All clinical indexes in the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).one day after surgery,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,while CD8^(+)was lower than than in the control group,the differences were statistically significant(P<0.05).Two months after surgery,the lung function indexes of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).The complication rate of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of single-hole thoracoscopic approach has more obvious advantages in improving the therapeutic effect of patients with early non-small cell lung cancer,which can reduce the immune damage suffered by patients,improve their lung function and reduce the incidence of complications.
