右美托咪定辅助椎管内麻醉的镇静效果观察

Observation on Sedative Effect of Dexmedetomidine Assisted Intraspinal Anesthesia

目的:分析右美托咪定辅助椎管内麻醉的镇静效果。方法:选取2021年11月—2022年11月黔东南州中医医院收治的椎管内麻醉患者68例为研究对象,根据随机数字表法划分为试验组和对照组,各34例。对照组实施常规椎管内麻醉方案,试验组在对照组基础上联合右美托咪定辅助麻醉。比较两组患者麻醉效果、镇静效果、各项时间指标及不良反应。结果:试验组呼吸频率、收缩压、舒张压及心率水平低于对照组,差异具有统计学意义(P=0.00);泵注右美托咪定15 min、泵注右美托咪定30 min、泵注右美托咪定60 min、手术结束时,试验组Ramsay镇静评分低于对照组,差异具有统计学意义(P=0.00);试验组住院时间短于对照组,差异具有统计学意义(P=0.00);试验组躁动、胸闷及恶心呕吐发生率低于对照组,差异具有统计学意义(P<0.05)。结论:患者在手术中,采用椎管内麻醉联合右美托咪定辅助方案,能够提高镇静效果,降低术后不良反应发生率,保障患者手术诊疗的稳定性和安全性。

Objective:To analyze the sedative effect of dexmedetomidine assisted intraspinal anesthesia.Methods:A total of sixty-eighty patients with intraspinal anesthesia admitted to Qiandongnan Prefecture Hospital of Traditional Chinese Medicine from November 2021 to November 2022 were selected as the study objects,and were divided into experimental group and control group according to random number table method,with thirty-four cases in each group.The control group was subjected to conventional intraspinal anesthesia,and the experimental group was combined with dexmedetomidine.The anesthetic effect,sedation effect,various time indexes and adverse reactions were compared between the two groups.Results:The respiratory rate,systolic blood pressure,diastolic blood pressure and heart rate of experimental group were lower than those of control group,and the differences were statistically significant(P=0.00).After injection of dexmedetomidine for 15 min,dexmedetomidine for 30 min and dexmedetomidine for 60 min,the sedation score of the experimental group was lower than that of the control group at the end of operation,and the difference was statistically significant(P=0.00).The duration of hospitalization in the experimental group was shorter than that in the control group,and the difference was statistically significant(P=0.00).The incidence of agitation,chest tightness,nausea and vomiting in experimental groups was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion:Intravertebral anesthesia combined with dexmedetomidine can improve the sedation effect,reduce the incidence of postoperative adverse reactions,and ensure the stability and safety of surgical diagnosis and treatment.
