血清t-PSA、f-PSA、f-PSA/t-PSA比值及铁蛋白检测在前列腺癌诊断中的临床价值

The Clinical Value of Serum t-PSA,f-PSA,f-PSA/t-PSA Ratio and Ferritin Detection in the Diagnosis of Prostate Cancer

目的:探析血清总前列腺特异性抗原(t-PSA)、游离前列腺特异性抗原(f-PSA)、f-PSA/t-PSA及铁蛋白检测在诊断前列腺癌(PC)中的应用效果。方法:选取2021年2月—2023年1月盘锦市中心医院收治的疑似PC患者148例为研究对象,按照病理诊断结果分为PC组71例与前列腺增生组77例。所有研究对象均进行t-PSA、f-PSA、铁蛋白检验,并将临床病理诊断的结果作为金标准,计算、对比单一检验和联合检验在诊断PC的诊断效能,t-PSA、f-PSA、f-PSA/t-PSA、铁蛋白水平。结果:联合检验诊断PC的准确率、特异度、敏感度、阳性预测值、阴性预测值均高于单一的t-PSA、f-PSA、f-PSA/t-PSA、铁蛋白检验,差异具有统计学意义(P<0.05);PC组t-PSA、f-PSA、铁蛋白水平高于正常前列腺增生组,f-PSA/t-PSA低于前列腺增生组,差异具有统计学意义(P<0.001)。结论:t-PSA、f-PSA、f-PSA/t-PSA、铁蛋白检验均可用于PC的诊断中,但实施联合检验能够提升诊断效能,临床应用价值高,建议采纳。

Objective:To explore the application effect of serum total prostate-specific antigen(t-PSA),free prostate-specific antigen(f-PSA),f-PSA/t-PSA and ferritin detection in the diagnosis of prostate cancer(PC).Methods:One hundred and forty-eight suspected PC patients admitted to Panjin Central Hospital from February 2021 to January 2023 were selected as the study objects.According to the results of pathological diagnosis,they were divided into PC group with seventy-one cases and benign prostatic hyperplasia group with seventy-seven cases.t-PSA,f-PSA,and ferritin tests were performed on all subjects.The results of clinicopathological diagnosis were taken as the gold standard,and the diagnostic efficiency of single test and combined test in the diagnosis of PC,t-PSA,f-PSA,f-PSA/t-PSA,and ferritin levels were calculated and compared.Results:The accuracy,specificity,sensitivity,positive predictive value,and negative predictive value of the combined test for diagnosing PC were higher than those of the single t-PSA,f-PSA,f-PSA/t-PSA,and ferritin tests.The difference was statistically significant(P<0.05).The levels of t-PSA,f-PSA and ferritin in the PC group were higher than those in the normal benign prostatic hyperplasia group,and the f-PSA/t-PSA was lower than that in the benign prostatic hyperplasia group,the difference was statistically significant(P<0.001).Conclusion:t-PSA,f-PSA,f-PSA/t-PSA,and ferritin tests can all be used in the diagnosis of PC,but the implementation of combined testing can improve the diagnostic efficiency and has high clinical application value.Therefore,it is recommended to be adopted.
