摘要
目的 研究外周血白细胞端粒相对长度与老年性聋的关系,探讨端粒长度与老年性聋患者人工耳蜗植入(CI)效果之间是否存在相关性。方法 选择65例无其他听力损失原因的老年性聋患者设为实验组,52名听力正常的老年人作为对照组。利用外周血白细胞DNA端粒重复序列拷贝数与内参基因拷贝基因数的比值(t/s比值),比较两组端粒长度,分析端粒长度与年龄的关系。其中28例重度/极重度老年性聋患者行CI,记录患者术前、术后6个月、术后12个月的听觉行为分级、言语可懂度分级、安静状态下言语识别率,并分析与端粒长度的关系。结果 老年性聋患者的端粒相对长度较对照组短,且外周血白细胞端粒长度随年龄的增长而下降,差异具有统计学意义(P均<0.001),老年性聋CI患者术后6个月、术后12个月的听觉行为分级、言语可懂度分级、安静情况下言语识别率较术前显著提高(P<0.01),端粒长度与言语可懂度分级、安静情况下言语识别率显著相关,与听觉行为分级相关性不显著。结论 外周血白细胞端粒长度可能是年龄相关性听力损失的潜在生物标志物,并且对老年性聋患者CI的疗效具有预测价值。
OBJECTIVE To investigate the correlation between telomere length in peripheral blood leukocytes and presbycusis,and to explore whether there is a correlation between telomere length and the efficacy of cochlear implantation in presbycusis patients.METHODS Sixty-five patients with senile deafness without other causes of hearing loss were selected as the experimental group,and 52 patients with normal hearing were selected as the control group.The telomere length of the two groups was compared by the ratio of the copy number of the DNA telomere repeat sequence to the copy number of the internal reference gene(t/s ratio),and the relationship between telomere length and age was analyzed.Among them,28 patients with severe and extremely severe presenile deafness received cochlear implantation.The auditory classification criteria,speech intelligibility rating and mandarin hearing in quiet condition were recorded before surgery,6 months and 12 months after surgery,and the relationship between surgical outcome and telomere length was analyzed.RESULTS The relative telomere length of presbycusis patients was shorter than that of control group,and the telomere length of peripheral blood leukocytes decreased with the increase of age,the difference was statistically significant(P<0.001).The auditory classification criteria,speech intelligibility rating,and mandarin hearing in quiet condition were significantly increased in patients with presenile deafness after 6 months and 12 months after surgery(P<0.01).Telomere length was significantly correlated with speech intelligibility rating and mandarin hearing in quiet condition,but not significantly correlated with auditory classification criteria.CONCLUSION Telomere length of peripheral leucocytes may be a potential biomarker of age-related hearing loss,and has predictive value for the efficacy of cochlear implantation in patients with presbycusis.
