不同手术入路治疗累及咽旁间隙腮腺深叶肿瘤效果分析

Used different approaches for surgical treatment for deep lobe parotid tumor of the parolid gland invading the parapharyngeal space

目的:探讨不同手术入路治疗累及咽旁间隙腮腺深叶肿瘤的临床疗效。方法:收集28例累及咽旁间隙的腮腺深叶良恶性肿瘤患者病例。肿瘤部位:软腭咽侧壁隆起12例,颌后区隆起14例,2例在行颅脑CT检查时发现。根据术前MR确定肿瘤的部位、大小,形状及与周围组织的关系而采用不同的手术入路。结果:28例患者均顺利出院,伤口愈合良好,咬合关系正常。术后随访3~48月有1例复发。结论:咽旁间隙腮腺深叶肿瘤根据术前肿瘤的部位、大小,形状及与周围组织的关系,灵活采用不同手术入路可获得满意的临床效果。

Objective:To investigate the clinical efficacy different surgical approaches for tumors invading the parapharyngeal space of deep lobe of the parotid gland.Methods:A total of 28 patients with benign and malignant tumors of the deep lobe of the parotid gland invading the parapharyngeal space were collected.Tumor sites:12 cases of soft palate and pharyngeal wall bulge,14 cases of retromaxillary bulge,2 cases were found during cranial CT examination.According to the preoperative MR to determine the tumor location,size,shape and the relationship with the surrounding tissue and use the appropriate surgical approach.Results:All 28 patients were discharged from hospital successfully,with good wound healing and normal occlusal relationship.There was one recurrence during the follow-up period of 3~48 months postoperation.Conclusion:For the deep lobe tumor of the parotid gland in the parapharyngeal space,according to the location,size,shape and relationship with the surrounding tissues,different surgical approaches can be used flexibly to obtain satisfactory clinical results.
