摘要
目的:构建卓越现场管理模式,探讨其在耳鼻喉专科手术器械管理中的应用价值。方法:卓越现场管理模式采用调研和抽查的形式分析耳鼻喉专科手术器械管理中存在的问题,分层组建管理团队,在实施卓越现场管理的基础上进行日常监督和考核反馈,动态优化管理目标和管理计划。选取2021-2022年医院耳鼻喉专科临床在用的230台(套)手术医疗器械,根据管理模式不同,将其分为对照组和观察组,对照组采用传统管理模式,观察组采用卓越现场管理模式,对比两组手术室现场器械的处理有效性、整合规范度、流程差错率和医护人员对管理质量的满意度。结果:观察组每月无关器械数量为(3.50±1.88)台(套),低于对照组,其差异有统计学意义(t=7.238,P<0.05);观察组通用医疗设备、专用医疗设备和手术治疗器具的整合规范性评分分别为(90.92±5.77)分、(93.26±3.90)分和(93.79±4.09)分,均高于对照组,其差异有统计学意义(t=3.457,t=4.294,t=5.367;P<0.05);观察组手术器械的备用检查、使用操作、回收清洗及维护维修差错率分别为6.7%(4/60)、3.3%(2/60)、0%(0/60)和10.0%(6/60),均低于对照组,其差异有统计学意义(x^(2)=4.615,x^(2)=4.904,x^(2)=6.316,x^(2)=4.675;P<0.05);医护人员对观察组现场管理质量的满意度为(92.37±5.61)%,高于对照组,其差异有统计学意义(t=5.274,P<0.05)。结论:卓越现场管理模式能减少耳鼻喉专科手术室现场无关器械摆放,规范器械存放位置和标识,避免管理流程的工作差错,提高临床服务质量。
Objective:To construct an excellent field management mode,and to explore its application value in the management of otolaryngology surgical instruments.Methods:The excellent field management mode adopted the form of survey and spot check to analyze the problems existing in the management of otolaryngology surgical instruments.Management teams were established in different levels,daily supervision and assessment feedback were carried out on the basis of the implementation of excellent field management,and management objectives and management plans were dynamically optimized.A total of 230 surgical instruments(sets)in clinical use in otolaryngology department of the hospital were selected and divided into control group and observation group according to different management modes.The control group adopted traditional management and the observation group adopted excellent field management mode.The processing effectiveness,integration standardization,process error rate and service satisfaction of the two groups of field instruments in the operating room were compared between the two groups.Results:The monthly number of irrelevant devices in the observation group was(3.50±1.88),which was lower than that in the control group,and the difference between groups was statistically significant(t=7.238,P<0.05).The integrated normative scores of general medical equipment,special medical equipment and surgical treatment equipment in the observation group were(90.92±5.77)points,(93.26±3.90)points and(93.79±4.09)points respectively,which were higher than those in the control group,and the difference between groups was statistically significant(t=3.457,t=4.294,t=5.367,P<0.05).The error rates of standby inspection,use operation,recovery cleaning and maintenance of surgical instruments in the observation group were 6.7%(4/60),3.3%(2/60),0%(0/60),and 10.0%(6/60)respectively,which were lower than those in the control group,and the difference between groups was statistically significant(x^(2)=4.615,x^(2)=4.904,x^(2)=6.316,x^(2)=4.675,P<0.05).The satisfaction of medical staff on the field management quality of the observation group was(92.37±5.61)%,which was higher than that of the control group,and the difference between groups was statistically significant(t=5.274,P<0.05).Conclusion:Excellent field management mode can reduce the placement of irrelevant instruments in the operating room of otolaryngology specialty,standardize the storage location and identification of instruments,avoid the work errors in the management process,and improve the quality of clinical services.
