89株纹带棒状杆菌耐药性及其耐药基因特征分析

摘要 目的通过分析河北省华北理工大学附属医院临床分离的纹带棒状杆菌的耐药性和基因组特征,为临床鉴别、治疗和防控提供参考。方法收集并鉴定临床分离非重复纹带棒状杆菌89株。经VITEK2-Compact全自动细菌仪联合16S rRNA鉴定为纹带棒状杆菌,采用微量肉汤稀释法进行体外药敏试验,使用全基因组测序技术进行测序。结果药敏结果显示所有菌株对利奈唑胺及万古霉素都表现为敏感,对环丙沙星(89,100.0%)全部耐药;全基因组测序共检出identity在90%以上耐药基因16种,大环内酯类和氨基糖苷类耐药基因和耐药表型匹配度较高;在基因组水平上,纹带棒状杆菌的传播具有优势克隆群。结论该院分离的纹带棒状杆菌同时携带多种抗菌药物的耐药基因,其某些耐药表型和耐药基因型具有一致性。该院纹带棒状杆菌院内传播由于优势克隆群的存在,菌株在进化过程中能通过耐药基因的获得及累积而产生不同的克隆群。 Objective The drug resistance and genomic characteristics of Corynebacterium striatum isolated from the North China University of Science and Technology Affiliated Hospital in Hebei Province were analyzed to provide references for clinical identification,treatment,prevention,and control.Methods Eighty-nine clinically isolated strains of non-repeating Corynebacterium striatum were collected and identified.Corynebacterium striatum was identified by VITEK2-Compact automatic bacterial instrument combined with 16S rRNA.In vitro drug sensitivity test was performed by the broth dilution method,and whole genome sequencing technology was used for sequencing.Results All strains were sensitive to linezolid and vancomycin,and resistant to ciprofloxacin(89,100.0%).A total of 16 drug resistance genes with identity above 90%were detected by whole-genome sequencing,and the drug resistance genes of macrolides and aminoglycosides had a high matching degree with drug resistance phenotypes.At the genomic level,the spread of Corynebacterium striatum had dominant clonal populations.Conclusion Corynebacterium striatum isolated from our hospital carried multiple antibiotic resistance genes simultaneously,and its drug resistance phenotype and drug resistance genotype were highly consistent.Due to the existence of dominant clonal groups,corynebacterium striatum could produce different clonal groups through the acquisition and accumulation of drug resistance genes in the process of evolution.
