鼻咽癌放疗同期奈达铂每周方案化疗与3周方案化疗的耐受性对比研究

A Comparative Study on the Tolerability of Weekly Regimen Chemotherapy with Nedaplatin versus 3-week Regimen for Nasopharyngeal Carcinoma Radiotherapy Concurrently

目的观察鼻咽癌放疗同期奈达铂每周方案化疗与每3周方案化疗的急性毒性反应,比较其耐受性。方法回顾性选取2019年3月—2021年2月在赣州市肿瘤医院放疗科住院的局部晚期鼻咽癌患者66例的临床资料随机分为奈达铂每周组(A组,n=34)与奈达铂3周组(B组,n=32),两组均采用调强放疗,对两组的胃肠道反应、口腔黏膜反应、骨髓抑制、皮肤反应、口干、体质量下降情况进行观察和比较。结果两组均按期完成调强放疗及同期化疗,两组中骨髓抑制、胃肠道反应、颈部皮肤反应、口腔黏膜反应发生率相比,差异无统计学意义(P>0.05);A组体质量下降的发生率显著高于B组(66.7%vs 40.0%),差异有统计学意义(χ^(2)=4.214,P<0.05)。期间所有不良反应经治疗后均可缓解。结论鼻咽癌放疗同期奈达铂每周方案与每3周方案化疗的急性不良反应发生率无明显差异,患者均能耐受,但3周方案的体质量下降较轻,生活质量好。

Objective To observe the acute toxic reactions and compare the tolerability of nedaplatin weekly regimen chemotherapy with every 3-week regimen chemotherapy during the same period of radiotherapy for nasopharyngeal carcinoma.Methods The clinical data of sixty-six patients with locally advanced nasopharyngeal carcinoma hospitalized in the radiotherapy department of Ganzhou Cancer Hospital from March 2019 to February 2021 were retrospectively selected and randomly divided into nedaplatin weekly group(group A,n=34)and nedaplatin three-week group(group B,n=32).Intensity-modulated radiotherapy was used in both groups,and the gastrointestinal reactions,oropharyngeal mucosal reactions,bone marrow suppression,skin reactions,dry mouth,and weight loss were observed and compared between the two groups.Results Both groups completed the intensity-modulated radiotherapy and concurrent chemotherapy on schedule,and there was no statistically significant difference in the incidence of bone marrow suppression,gastrointestinal reactions,skin reactions in the neck and oral mucosal reactions in the two groups(P>0.05).The incidence of weight loss in group A was significantly higher than that in group B(66.7%vs 40.0%),and the difference was statistically significant(χ^(2)=4.214,P<0.05).All adverse reactions during the period were alleviated by treatment.Conclusion There was no significant difference in the incidence of acute adverse reactions between the weekly regimen of nedaplatin and every 3-week regimen of chemotherapy during the same period of radiotherapy for nasopharyngeal carcinoma,which were tolerated by patients,but the 3-week regimen was associated with less weight loss and better quality of life.
