非脱垂子宫良性病变行不同全子宫切除方式的疗效比较

Comparison of the Efficacy of Different Total Hysterectomy Modalities for Non-prolapsed Benign Uterine Lesions

目的比较分析非脱垂子宫良性病变患者行不同全子宫切除方式治疗的疗效。方法方便选取扬州市江都人民医院2021年12月—2022年12月收治的非脱垂子宫良性病变患者96例,根据手术切除方式不同将患者分成两组,组别为对照组与研究组,每组48例。对照组应用经腹全子宫切除术治疗,研究组应用腹腔镜下全子宫切除术治疗,观察比较两组患者围术期相关指标、临床疗效、术后疼痛程度、并发症发生情况。结果研究组术中出血量、下床活动时间、导尿管留置时间、肛门排气时间、住院时间优于对照组,差异有统计学意义(P<0.05)。研究组临床总有效率高于对照组,差异有统计学意义(P<0.05)。研究组术后视觉模拟评分法(VAS)分值(2.58±0.63)分低于对照组(3.03±0.65)分,差异有统计学意义(t=3.444,P=0.001)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论非脱垂子宫良性病变患者应用腹腔镜下全子宫切除术治疗的疗效确切,能够显著减轻患者疼痛程度,减少患者术后并发症的发生。

Objective To compare and analyze the efficacy of different total hysterectomy modalities in patients with benign non-prolapsed uterine lesions.Methods Ninety-six patients with benign non-prolapsed uterine lesions admitted to Yangzhou Jiangdu People's Hospital from December 2021 to December 2022 were conveniently selected,and the patients were divided into two groups according to different surgical resection methods,namely the control group and the study group,with 48 patients in each group.The control group was treated with transabdominal total hysterectomy,and the study group was treated with laparoscopic total hysterectomy.The perioperative indexes,clinical efficacy,postoperative pain and complications were observed and compared in the two groups.Results The amount of bleeding during operation,the time of getting out of bed,the retention time of Foley catheter,the time of anal exhaust,and the hospital stay in the study group were better than those in the control group,and the difference was statistically significant(P<0.05).The total clinical effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).The postoperative visual analogue scale(VAS)score of the study group(2.58±0.63)points was lower than that of the control group(3.03±0.65)points,and the difference was statistically significant(t=3.444,P=0.001).The incidence of complications in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The efficacy of laparoscopic total hysterectomy in patients with benign non-prolapsed uterine lesions is definite,which can significantly reduce the pain level and the occurrence of postoperative complications in patients.
