摘要
目的对慢性肾脏病严重程度与维生素D水平及炎症反应的相关性进行研究。方法选取上饶市人民医院2019年6月至2022年12月收治的82例慢性肾脏病患者,依据肾小球滤过率将其分成慢性肾脏病3~4期组(L组)、慢性肾脏病5期组(D组);另外选取14例健康体检者作为对照组。以各组血清维生素D的表达、炎症因子水平为对比项目,并对维生素D水平和肿瘤坏死因子(TNF-α)、白介素-6(IL-6)、高敏C反应蛋白(hs-CRP)以及白介素-8(IL-8)水平的相关性进行研究。结果L组的25羟维生素D[25(OH)D]水平以及TNF-α、IL-6、hs-CRP、IL-8等炎症因子表达水平均低于D组(P<0.05);L组与D组的25(OH)D水平(8.49±0.56)μg/L与(11.35±1.27)μg/L明显低于对照组的(35.24±4.25)μg/L(P<0.05);L组与D组的各项炎症因子表达水平也显著高于对照组(P<0.05);维生素D水平与炎症因子水平与慢性肾脏病严重程度之间呈正相关(P<0.05)。结论慢性肾脏病患者严重程度与维生素D水平及炎症反应之间密切相关,可为慢性肾脏病疾病的诊断、预防以及治疗提供重要依据。
Objective To study the relationship between the severity of chronic kidney disease(CKD)and vitamin D level and inflammatory response.Methods A total of 82 patients with CKD admitted to Shangrao People's Hospital from June 2019 to December 2022 were selected and divided into CKD 3-4 group(L group)and CKD5 group(D group)according to eGFR.Another 14 healthy subjects were selected as the control group.The serum vitamin D expression and inflammatory factor levels in each group were compared,and the correlation between vitamin D level and TNF-α,IL-6,HS-CRP and IL-8 levels was studied.Results The level of 25(OH)D and the expression of TNF-α,IL-6,Hs-CRP,IL-8 and other inflammatory factors in L group were lower than those in D group(P<0.05).The level of 25(OH)D in L and D groups(8.49±0.56)μg/L and(11.35±1.27)μg/L was significantly lower than that in control group(35.24±4.25)μg/L(P<0.05).The expression levels of inflammatory factors in L and D groups were significantly higher than those in control group(P<0.05).Vitamin D levels and inflammatory factors were positively correlated with the severity of CKD(P<0.05).Conclusion The severity of CKD patients is closely related to vitamin D level and inflammatory response,which can provide important basis for the diagnosis,prevention and treatment of CKD disease.
